摘    要:目的:探讨磁共振扩散加权成像结合表观扩散系数(ADC)值对鉴别良恶性卵巢上皮囊实性肿瘤的价值。方法:对经手术病理证实的127例卵巢上皮起源的囊实性肿瘤患者的常规MRI、DWI检查资料和肿瘤实体部分的ADc测量值进行回顾性分析,131个病灶中46个为良性囊腺瘤,85个为恶性囊腺瘤。结果:良性组卵巢肿瘤实质部分的平均ADC值为(1.69±0.25)×10-3mm2/s,恶性组为(1.03±0.22)×10-3mm2/s,两组间ADC值差异有极显著性意义(P〈0.001)。以(1.25×10-3)mm2/s作为诊断良恶性肿瘤的阈值,则敏感度、特异度和总体符合率分别为90.1%、89.9%和92.4%。结论:常规MRI检查鉴别卵巢表面上皮良恶肿瘤存在困难时,辅以扩散加权成像和ADC值测量有助于进一步判断肿瘤的良恶性。

Diffusion-weighted imaging in differenting the nature of ovarian epithelial tumors with solid and cystic components

Abstract:Objective:To assess the usefulness of diffusiowweighted imaging (DWI) with apparent diffusion coeffi- cient (ADC) value measurement in differentiating benign from malignant ovarian epithelial tumors with cystic and solid components. Methods: The conventional MR imaging (MRI),DWI and the ADC values of solid components of 131 ovarian epithelial tumors confirmed by surgery and pathology were retrospectively analyzed. Of the 131 tumors, there were 46 be- nign and 85 malignant cystadenomas. The ADC values of solid components were calculated and analyzed statistically. Results:The mean ADC value of solid components in benign ovarian epithelial tumors was (1.69±0.25)×10-3mm2/s, in malignant ovarian epithelial tumors was ( 1. 03 ± 0. 22 )×10-3 mm2 /s, with significant statistic difference (P 〈 0. 001 ). Taking the ADC value of (1. 25×10-3) mm2/s as the threshold value for differentiation between benign and malignant ovarian epithelial tumors, the sensitivity, specificity and accuracy was 90.1%, 89.9 % and 92.4 % respectively. Conclusion : When differentiation of benign or malignant ovarian epithelial tumors was difficult hy using conventional MRI,DWI and measurement of ADC values of solid components might be helpful for further evaluation.
