老年结肠癌根治术后并发症发生的影响因素

目的：分析老年结肠癌根治术后并发症发生的影响因素，并探讨预防对策。方法对133例接受手术治疗的老年结肠癌患者的临床资料进行回顾分析，按照其并发症发生情况进行分组探究。结果133例老年结肠癌患者中37例（27．8％）出现并发症，其中出血2例，感染25例，栓塞形成10例。单因素回归分析发现，2组患者手术时间、ASA分级、APACHE-Ⅱ评分、肿瘤分化程度及合并症存在统计学差异（ P＜0．05）；多因素回归分析发现，手术时间≥150 min、ASA分级Ⅲ～Ⅳ级、APACHE-Ⅱ评分≥14分、肿瘤中低分化及合并糖尿病是老年结肠癌根治术后并发症发生的独立危险因素（P＜0．05）。结论上述危险因素提示患者并发症风险极高，对于这一类患者，应及时采取相关预防措施，以降低并发症发生率、保证其预后和生存质量。

Influence Factors for Complications of Elderly Patients with Colon Cancer after Radical Resection

Objective To analyze the influence factors for complications of elderly patients with colon cancer after radi-cal resection,and to explore the preventive countermeasures.Methods A retrospective analysis of 133 cases of elderly patients with colon cancer treated with operation were conducted,and in accordance with complications were divided into 2 groups.Results In 133 cases of elderly patients with colon cancer,there were 37 cases (27.8%) with complications,including 2 cases of hemor-rhage,25 cases of infection,and 10 cases of thrombosis.Single factor regression analysis showed that there had statistical differ-ence between the 2 groups in operation time,ASA score,APACHE-score,degree of tumor differentiation and complications ( P<0.05);multi-factor regression analysis showed that operation time≥150 min,ASA gradeⅢ~Ⅳ,APACHE-Ⅱ≥14,middle and low tumor differentiation,diabetes were independent risk factors for complications of elderly patients with colon cancer after radical resection (P<0.05).Conclusion The risk factors of patients indicate high risk for complications.For this type of patients, timely preventive measures should be taken in order to reduce the complication rate,ensure the prognosis and quality of life.
