三维适形放疗联合化疗同步治疗非小细胞肺癌晚期局部复发的近期疗效

目的：探讨三维适形放疗联合化疗同步治疗非小细胞肺癌晚期局部复发的近期疗效。方法选择非小细胞肺癌晚期局部复发患者80例为研究对象。按照随机数表法，将患者平均分为治疗组和对照组。治疗组肿瘤患者采用三维适形放疗，联合使用多西紫杉醇。对照组单纯使用三维适形放疗。治疗结ss束两个月后对患者的治疗疗效进行评价，观察两组患者不良反应发生情况。对所有患者随访2年。观察记录两组患者1年、2年生存率。分析比较两组患者的死亡原因。结果治疗组患者治疗总有效率为87．50％，对照组患者治疗中有效率为57．50％，两组患者治疗总有效率差异显著（P＜0．05）。两组患者均出现不同程度的不良反应，放射性食道炎、胃肠道反应、骨髓抑制、放射性肺炎。治疗组患者的不良反应率为2．25％，明显低于对照组的不良反应率（77．50％），两组患者不良反应率比较差异显著（P＜0．05）。治疗组患者1年死亡率为32．350％，明显低于对照组的1年死亡率（70．00％），两组患者1年死亡率差异显著（P＜0．05）。治疗组患者2年生存率52．50％，明显高于对照组的2年生存率（30．00％），两组患者2年生存率差异具有统计学意义（P＜0．05）。两组患者具有相同的死亡原因，局部未控、远处转移、放射性肺损伤并感染等其他原因。两组患者在死亡原因方面比较无显著性差异（P＞0．05）。结论三维适形放疗同步化疗方法治疗的非小细胞肺癌晚期局部复发患者，近期治疗效果明显，治疗后不良反应发生率低，患者生存率高，死亡率低。值得在临床治疗非小细胞肺癌晚期局部复发患者过程中推广使用。

Short-term Efficacy of Three-dimensional Conformal Radiotherapy Combined with Chemotherapy in the Treatment of Local Recurrence in Advanced Non-small Cell Lung Cancer

Objective To investigate the three-dimensional conformal radiotherapy combined with chemotherapy for lo-cally recurrence in advanced non-small cell lung cancer.Methods 80 cases with advanced non-small cell lung cancer were se-lected for the study.According to random number table method,the patients were divided into the treatment group and the control group.The treatment group received three-dimensional conformal radiotherapy combined with paclitaxel chemotherapy.The control group received three-dimensional conformal radiation treatment.After two months of treatment,efficacy for treatment of patients e-valuated,the incidence of adverse reactions were observed.All patients were followed up for 2 years.Results The total effective rate in the treatment group was 87.50%,and 57.50%in the control group,the total efficiency between the 2 groups was signifi-cantly different (P<0.05).All patients with varying degrees of adverse reactions,radioactive esophagitis,gastrointestinal reac-tions,bone marrow suppression,radiation pneumonitis.Adverse reactions in the treatment group was 2.25%,significantly lower than 77.50%in the control group,there had significant difference (P<0.05).1-year mortality rate in the treatment group was 32.35%,significantly lower than 70.00%in the control group,there had significant difference ( P<0.05) .2-year survival rate in the treatment group was 52.50%,significantly higher than 30.00%in the control group,there had significant difference (P<0.05).Both groups had the same cause of death,local failure,distant metastasis,radiation-induced lung injury and infection,and other causes.There had no significant difference in causes of death between the 2 groups ( P>0.05) .Conclusion Three-di-mensional conformal radiotherapy combined with chemotherapy for locally recurrence in advanced non-small cell lung cancer has obvious effect,incidence of adverse reactions is low after treatment,the survival rate of patients is high,and the mortality rate is low.It is worthy of clinical application.
