电视胸腔镜辅助手术治疗自发性气胸的临床体会

目的探讨电视胸腔镜辅助手术治疗自发性气胸的手术方式方法回顾2008年5月-2010年11月收治的自发性气胸患者行电视胸腔镜辅助于腋中线第6或第7肋间作小切口治疗38例,并和同期进行常规手术治疗的自发性气胸患者38例进行比较。结果两组手术均成功,胸腔镜组手术时间90-190min,平均90±10.2min,切口长5-7cm,出血量40±5.2ml,术后胸管引流时间3-5d;常规组手术时间40-50min,平均45±8.8min,切口长15-20cm,出血量为50±3.7ml,术后胸管引流时间7-8d,两组患者均痊愈出院,随访6-9个月,无复发病例。结论电视胸腔镜辅助手术治疗自发性气胸具有损伤小,出血少,术后恢复快,疗效确切等优势。

Experience on Video-assisted Thoracoscopic Surgery in the Treatment of Spontaneous Pneumothorax

Objective Probe operational styles and clinical experience of video-assisted thoracoscopic surgery in the treatment of spontaneous pneumothorax is probed and gotten.Methods 38 Cases of patients with spontaneous pneumothorax were reviewed,who had undergone video-assisted thoracoscopic small incision operation made from May 2008 to November 2010 along the midaxillary line at the sixth or seventh intercostal,and put into comparison with other 38 cases of patients with spontaneous pneumothorax undergoing conventional operation during the same period.Results Both operations are successful.The operating time for video-assisted thoracoscopic surgery is ranged from 90-190 minutes(MEAN=90 minutes) with its incision long from 5-7cm and 40ml of bleeding volume.The time of chest drainage is ranged from 3 to 5 days after the operation;The operating time for the conventional operation is ranged from 40 to 50 minutes(MEAN=45 minutes) with its incision long from15-20cm and 50ml of bleeding volume.The time of chest drainage is ranged from 7 to 8 days after its operation;No relapse occurred during 6-9 month-long follow-up after the operations.Conclusions Video-assisted thoracoscopic surgery in the treatment of spontaneous pneumothorax has such advantages as fewer lesions,less bleeding,rehabilitating more quickly after the operation and exact curative effect.
