96例急性有机磷农药中毒患者心肌损害

目的探讨急性有机磷农药中毒患者心肌酶学及心电图变化,了解其对心肌损害情况。方法对96例急性有机磷农药中毒患者（轻度中毒组41例、中度中毒组34例、重度中毒21例）入院后24 h内抽取空腹静脉血测定肌酸激酶（CK）、肌酸激酶同工酶（CK-MB）、乳酸脱氢酶（LDH）、α-羟丁酸脱氢酶（HBDH）、天门冬氨酸氨基转移酶（AST）,并常规描记体表心电图。结果急性轻度有机磷农药中毒患者血清心肌酶除CK外,余指标与正常对照组无显著性差异（P〉0.05）;中、重度中毒患者较正常对照组明显增高（P〈0.01）;急性有机磷农药中毒患者心电图可出现窦性心动过速、ST-T改变、房性早搏等变化。结论急性中、重度有机磷农药中毒患者存在心肌损害,中毒程度越深,心肌损害越重;同时有机磷中毒可致不同类型心电图改变。

Myocardial damage in 96 patients with acute organophosphorus pesticide poisoning

Objective To investigate the change of myocardial enzyme and electrocardiogram in patients with acute organophosphorus pesticide poisoning（AOPP）,testing the damage state of myocardium.Methods Venous blood of 96 patients with acute organophosphorus pesticide poisoning were collected with empty stomach in 24 hours（mild poisoning cases 41,midrange poisoning cases 34,severe poisoning cases 21）to determine serum creatine kinase（CK）,lactate dehydrogenase（LDH）,α-hydroxybutyrate dehydrogenase（HBDH）,aspartate amino transferase（AST）,surface electrocardiograms were detected.Results Compared with the control group,there was no significant difference of myocardial enzyme in mild poisoning group except CK;there was a significant difference in midrange group and severe group compared with control group;sinus tachycardia,ST-T change,atrial premature beats and other changes could be found through electrocardiogram tracing.Conclusion Compared with the control group,there is no significant difference of myocardial enzyme in mild poisoning group except CK;there is a significant difference in midrange group and severe group compared with the control group;sinus tachycardia,ST-T change,atrial premature beats and other changes could be found through electrocardiogram tracing.
