下鼻甲等离子消融和部分切除的疗效比较

摘    要:目的评价鼻窦内窥镜术中下鼻甲肥厚部分切除术与等离子低温射频消融术的效果。方法将鼻窦内窥镜术中伴有下鼻甲肥厚者86例分为两组:实验组(A组)43例运用等离子低温射频消融下鼻甲,对照组(B组)43例用电动刨削器切除下鼻甲粘膜肥厚部分。术后2、6个月用VA S评分法评价两组患者的鼻塞程度差异,并对术后两组鼻甲粘膜的转归情况进行比较。结果组内治疗前后鼻塞程度的差异均有统计学意义;组间在治疗前和治疗后鼻塞程度差异均无统计学意义。术后两组之间在术腔清洁、粘膜转归、上皮化阶段有差别,实验组在相应时间内更容易恢复。结论鼻内镜术中采用等离子低温射频消融治疗肥厚的下鼻甲具有微创、高效、保护粘膜功能、术腔恢复快等特点。

Abstract:Objective To compare the effect of plasma radiofrequency ablation of inferior turbinates and inferior turbinectomy for patients with hypertrophic inferior turbinates.Methods Eighty-six cases of hypertrophic inferior turbinates were divided in two groups: 43 cases were treated by plasma radiofrequency ablation of inferior turbinates(Group A),and other 43 cases were treated by inferior turbinectomy.Follow-up lasted 2-6 months after surgery.The effect of the two measures was compared and the preoperative and postoperative nasal obstruction was investigated by visual analogue scale(VAS) scores between the two groups.Results VAS scores of nasal obstruction were decreased significantly after treatment in both groups,but there was no significant difference between the two groups after surgery.The time needed for recovery was shorter in Group A than B.Conclusion Plasma radiofrequency ablation is a minimally invasive,safe,effective method for treatment of inferior turbinate hypertrophy.
