156例婴幼儿法洛四联症根治术

目的 探讨婴幼儿期施行法洛四联症根治术的结果和经验。方法 对1994年1月至2001年12月连续施行156例3周岁以下法洛四联症根治术进行总结。年龄2-35个月，体重3．5-15．0kg。在中度低温体外循环下手术。除6例外，均采用自体心包补片修补室间隔缺损。采用自体心包补片扩大右心室流出道。结果 手术死亡16例，手术死亡率为10．3％，显著高于同期477例3-14岁小儿法洛四联症根治术的手术死亡率(1．89％)。手术死亡原因和术后并发症主要为低心排综合征和心律失常。结论 婴幼儿期法洛四联症根治术的手术死亡率仍较高，应进一步改进手术技术，加强围术期监护处理，以提高手术成功率。

Radical correction of Tetralogy of Fallot in infancy: Experiences of 156 cases

Objective To review the experiences of radical correction of Tetralogy of Fallot (TOF)in infants. Methods Between Jan. 1994 and Dec. 2001, 156 children underwent one stage correction of TOF. The age ranged from 2 to 35 months and weight ranged from 3.5 to 15.0 ?kg. The operations were carried out under medium hypothermic extracorporeal circulation. With exception of 6 cases, auto pericardium patch repair of ventricular septal defect was performed. Right ventricle outlet was also enlarged with pericardium.Results There were 16 operative deaths ( 10.25% ), which were significantly higher than 477 cases of children operated on the age from 3 to 14 years ( 1.89% ). The main cause of death was low cardiac output syndrome and arrhythmia.Conclusion The mortality rate of surgical treatment of TOF in infants is still keeping high, a further improvement in operative techniques and perioperative care should be enhanced.
