电镜及免疫标记髓过氧化物酶检测对急性白血病分型诊断的意义

摘    要:目的 比较电镜髓过氧化物酶(MPO)、免疫标记抗髓过氧化物酶抗体(抗MPO抗体)、光镜过氧化物酶(POX)对急性白血病分型诊断的敏感性和特异性。方法 对38例初诊的急性白血病患者,APAAP法检测MPO抗体及电镜MPO细胞化学染色。结果 光镜POX阳性组抗MPO及电镜MPO染色均呈阳性,光镜POX阴性组22例中有5例抗MPO抗体阳性,6例电镜MPO染色阳性。结论 抗MPO法及电镜化染对急性白血病分类中髓系特征的判断具有敏感性好,特异性高的优点,个别病例电镜MPO反应较免疫标记MPO抗体更为有效和敏感。

Abstract:Objective Comparison on the sensitivity and specifity of MPO Electron microscopy,immuno-labeled MPO antibody and light microscopy to the classification of acute leukemia.Methods APAAP Determination of MPO antibody and MPO electron microscopy for the 38 cases of acute leukemia patieuts( for FAB classification,20cases were ALL,16 cases were ANLL,and 2 cases could not be classitied.Results For the POX positive group detected by light microscopy, positive results were obtained by both immuno-labeled MPO antibody and MPO electron microscopy;for the POX negative group (22 cases) detected by light microscopy,5 cases were detected as positive by immuno-labeled MPO antibody and 6 cases were positive by MPO electron microscopy.Conclusions Both immuno-labeled MPO antibody and MPO electron microscopy are sensitive and specific to the diagnoses of myeloid characterization in the classification of acute leukemia.In few cases,MPO electron microscopy is more effective and more sensitive than immuno-labeled MPO antibody method.
