ICU肺部铜绿假单胞菌感染的药物敏感监测

目的:研究ICU引起肺部感染的铜绿假单胞菌对常用抗菌药物的敏感性,指导临床用药。方法:收集2003年1月至2003年12月本院ICU患者下呼吸道分泌物进行细菌培养,采用VITEK2ID-GNB系统鉴定菌种及药物敏感试验。结果:共分离出33株铜绿假单胞菌,其药物敏感度依次为丁胺卡那霉素78.8%,哌拉西林/他唑巴坦69.7%,环丙沙星、亚胺培南、左旋氧氟沙星、妥布霉素均为66.7%,庆大霉素60.6%,哌拉西林57.6%,头孢他定42.4%;第一、二代头孢菌素高度耐药。结论:我院ICU铜绿假单胞菌对氨基糖甙类、喹诺酮类,碳青霉烯类,酰脲类青霉素有较高敏感性;对大部分其它青霉素类抗菌素,第一、二代头孢菌素及部分第三代头孢菌素有较高耐药性。

Monitoring of Drug-susceptibility of Pulmonary Infection Caused by Pseudomonas Aeruginosa in ICU

Objective: To investigate the drug resistance of the Pseudomonas aeruginosa which cause pulmonary infection in ICU, and to guide our rational drug administration in clinical practice. Methods: the secreta from lower respiratory tracts of the patients in ICU of our hospital were collected and cultivated, from January 2003 to December 2003. These bacteria were identified and their susceptibility was detected by VITEK2 ID-GNB system. Results: 33 plants of the Pseudomonas aeruginosa were isolated. Their drug-susceptibility to the antibiotics was 78.8% (Amikacin), 69.7% (Piperacillin-Tazobactam), 66.7%(Ciprofloxacin, Imipenem, Levofloxacin, Tobramycin), 60.6% (Gentamycin), 57.6% (Piperacillin), 42.4% (Ceftazidime); the first and the second generation cephalosporins had high drug-resistant rates. Conclusions: The Pseudomonas aeruginosa in ICU of our hospital had higher drud-susceptibility to aminoglycosides, quinolones, carbopenems, ureide derivative penicillins; higher drug-resistance to the other penicillins, the first and the second and some third generation cephalosporins.
