当前农村肺结核病人费用负担分析

目的：分析农民认识和肺结核病人实际费用支出，了解疾病造成的经济负担，为提高服务利用和结核病控制提供信息和依据。方法：定量与定性结合。结果：农民普遍估计诊治肺结核病所需费用是病人实际费用的2-5倍，肺结核病人实际费用支出占家庭年总支出的13％-40％，还有一定的间接费用。结论：国家为肺结核病人提供了免费诊断和治疗，但病人自负的费用仍较高，影响了就医行为和病情的及时发现和诊治。加强宣传、提高服务质量、规范医疗行为、为贫困地区提供更多的政策倾斜，将会加速结核病控制进程。

Analysis on health expenditure of pulmonary tuberculosis in rural areas

Objectives:Analyzing the health expenditure to find out the economic burden for patient and family,and understand their health care seeking behavior.Methods:Both quantitative and qualitative methods were applied,TB patients and community residents were included.Results:The health expenditure from com- munity perception was fairly high,2-5 times as real cost paid by patients.The health expenditure accounts for 12.6-39.9% total expenditure of their family.Besides high direct cost,there were some amount of indirect cost and the cost prior to diagnosis.Conclusions:Over estimating the health expenditure for TB diagnosis and treatment and higher economic burden whatever free-charge,it impacted health care seeking behavior and TB case detection,diagnosis and treatment.Health education and policy explanation,quality of services,norma- tive provider behavior and special support policy to poverty area need to be strengthened.
