血管端端吻合法建立双侧供肾大鼠肾移植模型

摘    要:目的:采用血管端端吻合法建立一种无需阻断主动脉和后腔静脉血流的双侧供肾大鼠肾移植模型.方法:取供体双侧肾脏,受体左侧肾窝单肾移植,生理盐水中血管端端吻合,输尿管膀胱瓣-膀胱外隧道式吻合行尿路重建.结果:40例移植肾肾动、静脉即时通畅率分别为85%(34/40)、90%(36/40),吻合血管无扭曲.模型并发症有吻合口出血、动静脉血栓形成、排斥反应等,不使用免疫抑制剂的情况下,90%受体存活时间超过7 d.结论:生理盐水中行血管端端吻合,可解决动脉吻合需阻断主动脉血流、静脉吻合需支架支撑、静脉粘贴缝合困难等问题,是一种简单、经济、省时的模型方法,一般实验室均可开展.

Abstract:Objective Using end-to-end vascular anastomosis technique,to set up a kidney transplantation model that the aorta and inferior vena cava are not interdicted in rats implanted with two kidneys from donor rats.Methods With technique of end-to-end vascular anastomosis in the normal saline,both kidneys were harvested from one donor and transplanted to two rats.Urinary tract reconstruction was accomplished by suturing the donor ureter with a bladder patch to the recipient bladder.Results Twenty SD rats were used as donors and 40 SD rats were used as recipients.Success rate of operation and immediately patency rate of vascular anastomosis stoma were 90%(36/40).Without using immunosuppressant,survival time of recipients was over 7 days.Conclusion Such problems as the adhesion of renal vein wall and difficult suture are solved by the technique of end-to-end vascular anastomosis in the normal saline.This modified rat model of kidney transplantation is simple,economical,reliable,stable with higher success rate and can be applied in the research of kidney transplantation.
