肾上腺皮质癌18例报告

目的 提高肾上腺皮质癌的诊治水平。方法 回顾分析18例肾上腺皮质癌的临床诊治资料。结果 18例中功能型肿瘤9例，平均存活22．8个月；无功能型肿瘤9例，平均存活8．12个月。手术完整切除9例，姑息性切除6例，穿刺活检3例。18例肿瘤中Ⅰ期3例，平均存活47．0个月，5年存活2例；Ⅱ期3例，平均存活37．0个月，3年存活2例；Ⅲ期4例，平均存活9．8个月；Ⅳ期8例，平均存活4．6个月。结论 手术是治疗肾上腺皮质癌唯一有效的方法，早期诊断、早期手术是治疗关键。

Adrenocortical carcinoma:a clinical study of 18 cases

Objective To evaluate the diagnosis and treatment ofadrenocortical carcinoma. Me-thods 18 cases of adrenocortical carcinoma were reviewed. Results In 50% of the patients (9/18),the tumor was functional,the median survival time being 22.8 months.In the other 9,the tumor was nonfunctional,the median survival time being 8.1 months.9 of the 18 underwent complete resection and 6 incomplete resection.Whereas,3 underwent needle biopsy only.The tumor was stage Ⅰ in 3 with the median survival time 47.0 months and the 5-year survival in 2.The tumor was stage Ⅱ in 3,the median survival time being 37 months and the 3-year survival in 2.4 were stage Ⅲ,the median survival time being 9.8 months and the 1-year survival in 2.8 were stage Ⅳ with the median survival time 4.6 months and the 2-year survival in 1. Conclusions Surgical ablation is the only treatment of choice.Early diagnosis and prompt surgery are important.
