超微创手术减少帕金森病术后并发症

目的 探讨超微创手术方法对减少帕金森病手术并发症的价值。方法 采用定位引导经皮钻颅的方法，仅作直径为4mm的骨孔，头皮不需缝针。结果 共施行微电极引导立体定向脑内核团毁损术治疗帕金森病150例，其中普通微创手术80例，超微创手术70例，手术疗效达90％以上，但在普通微创手术的80例中，共出现严重术后并发症5例，占6.3%，其中颅内出血4例（死亡1例，植物生存1例，重残1例，轻偏瘫1例），占5％，意外死亡1例，手术死亡率为2.5%。超微创手术的70例中，仅1例出现了毁损灶处少量出血（术后轻偏瘫，并在一周内恢复），占1.6%。结论 超微创手术方法缩短了手术时间，减少病人痛苦，并避免了术中脑脊液的丢失，可明显降低手术并发症的产生，同时也缩短了病人的住院时间，是一种值得提倡的方法。

Supermicroinvasive Operation to Reduce the Postoperative Complications of Parkinson''s Disease

Objective To evaluate the effect of supermicroinvasive method to reduce the complications of the surgical treatment of Parkinson's disease. Methods Just drill a small bone hall trans scalp with 4 mm in diameter without suture. Results 150 patients of Parkinson's disease were treated with microelectrode guided stereotactic deep brain nuclear coagulations. Ordinary microinvasive operations were in 80 cases, and supermicroinvasion in 70 cases. Total effective rate was above 90%. But severe complications were 5 cases(6.3%) in ordinary microinvasive operations. Include introcranial hematoma in 4 cases(5%)(1 case dead, 1 case vegetable state, 1 case severe disability, and 1 cases paralysis). In 70 supermicroinvasive operation cases, only 1 case(1.68%) appeared slight hematoma in coagulation area (slight paralysis postoperatively, and recovered in one week).Conclusion Supermicroinvasive method can reduce the operation period, the pain of patient, and avoid the lose of CSF during operation. It can reduce the complications significantly. In the same way, the days in hospital of the patients were reduced. It suggested that supermicroinvasive operation is quite a useful method.
