腹腔镜手术与单纯药物治疗盆腔炎的疗效比较

目的 比较腹腔镜手术与单纯药物治疗盆腔炎的疗效。方法 将1998年2月-2004年2月间123例盆腔炎患者分为单纯药物治疗组、腹腔镜治疗组和开腹治疗组。单纯药物组42例，腹腔镜组45例，开腹治疗组36例。治疗后随访12个月。结果手术治疗组治疗后盆腔炎性包块和盆腔痛发生率与单纯药物治疗组相比差异有显著意义（P〈0．005）。结论腹腔镜检查是诊断急性盆腔炎的较好方法。单纯药物治疗对已形成脓肿者效果不好。对无脓肿者有一定疗效，但复发率高。腹腔镜配合敏感抗菌药物是盆腔炎的最好治疗方法。

The effect of laparoscopic surgery on pelvic inflammatory disease

Objective To compare the effect of laparoscopic surgery and medicine alone on pelvic inflammatory disease(PID).Methods A total of 123 patients with PID,who treated in our hospital from February 1998 to February 2004,were divided into three groups: 42 in the group of medicine treatment alone,45 laparoscopic surgery group,and 36 open surgery group.All of the patients were for followed up for twelve months after treatment.Results Less patients with pelvic inflammatory masses and pelvic pain were found in the surgery groups than the medicine group(P<0.005).Conclusions Laparoscopy is a gold standard in diagnosis of PID.The effect of the medicine alone is poor in treating pelvic abscess.Although the medicine is effective for PID without abscess,the recurrence is high.Laparoscopic surgery combined with sensitive antibiotics is the best treatment of PID.
