止血贴压迫器在经桡动脉行心血管疾病介入术后患者中的应用

摘    要:目的 了解动脉止血贴压迫器在心血管疾病介入治疗后对桡动脉止血的效果.方法 2009年2月至2010年2月,第三军医大学大坪医院收治的经桡动脉路径行冠状动脉造影及介入治疗的患者610例,按入院先后将610例患者分为常规组(绷带加压包扎组)291例和观察组(动脉止血贴压迫器组)319例,所有患者均经桡动脉路径行冠状动脉造影及介入治疗,术后常规组采用绷带加压包扎止血,观察组采用动脉止血贴压迫器止血,观察并比较两组患者的止血时间、术侧手掌肿胀度及包扎处过敏情况等.结果 常规组患者的压迫止血时间为(7.7±0.3)min,观察组为(2.2士0.8)min,两组比较,差异有统计学意义(t=2.31,P<0.05);常规组患者的完全止血时间为(11.5±0.7)h,观察组为(3.4±0.6)h,两组比较,差异有统计学意义(t=1.97,P<0.05).两组患者术侧手掌的肿胀程度均以轻中度为主,均有1例发生重度肿胀,差异无统计学意义(u=7.59,P>0.05).常规组12例患者对绷带过敏,并有5例出现大小不等水泡,经处理后痊愈;观察组无一例出现过敏,差异有统计学意义(x2 =11.22,P<0.01).结论 使用动脉止血贴压迫器是一种较绷带加压包扎法更为安全、快捷的桡动脉止血方法.

Abstract:Objective To explore the application effects of haemostatic bandage compression device for patients treated by transradial coronary intervention operation.Methods Totally 610 patients after percutaneous coronary interventions were enrolled and divided into observational group (haemostatic bandage compression device group,n=319) and control group (haemostatic bandage group,n=291).Patients in the observational group were given haemostatic bandage compression device,and patients in the control group were given haemostatic bandage after transradial coronary intervention operation.Effects and elapsed time of haemostasis,oxyhemoglobin saturation in the operated hand,degree of hand swelling,incidence of allergy were observed and compared between the two groups.Results Compression time for the observational group and control group were (2.2±0.8)min and (7.7±0.3)min,respectively.Significant difference was found on compression time between the two groups(t=2.31,P<0.05).Oppression haemostatic time and completely stop bleeding time in haemostatic bandage compression device group (3.4 ± 0.6)h was much shorter than the haemostatic bandage group(11.5 ±0.7)h,(t =1.97,P<0.05).No significant difference was found in incidence of hand swelling in haemostatic bandage group the haemostatic bandage compression device group(u=2.59,P>0.05).No allergy was observed in observational group and 5 patients with blister among 12 allergy patients in the control group were found,and the difference was significant(x2=11.22,P<0.01).Conclusion Haemostatic bandage compression device in radial artery homeostasis after percutaneous coronary procedures was safer and easier than thetraditional haemostatic bandage.
