ICU患者腹腔内压力监测的重复性研究

目的 采用自行创建的腹腔内压力监测装置和方法测量危重患者的膀胱压力,通过观察监测者内部与监测者之间的测量数据变异性以评价腹腔内压力测定的可重复性.方法 2012年6-12月方便性抽样选择第二军医大学长征医院综合ICU 19名危重患者,采用自行创建的腹腔内压力监测装置进行腹腔内压力监测,对同一名测量者（监测者内部）和两名测量者（监测者之间）测定的数据进行Pearson相关分析.结果 Pearson相关分析显示监测者内部和监测者之间的膀胱压力测定数据相关性好,相关系数分别为0.791 （P＜0.01）和0.814（P＜0.01）,提示测量数据间强相关,无明显变异.结论 自行创建的腹腔内压力监测装置在测定腹腔内压力中具有确定的可重复性,采用标准的测定装置和测定方案可以在危重患者中可靠准确地测定腹腔内压力.

Repetitive Study of Intra-Abdominal Pressure Monitoring in Intensive Care Unit

Objective Bladder pressure was obtained by a selLdesigned intra-abdominal pressure（IAP） monitoring setting and methods,and this study evaluated the intramonitor and intermonitor variability of bladder pressure measurements and investigated the reproducibility of IAP measurement to ensure diagnostic accuracy.Methods By convenience sampling,a total of 19 critical patients in ICU from June to December,2012 were recruited and monitored by selLdesigned intra abdominal pressure（IAP） monitoring settings.Pearson＇s correlation analysis between the two measurers （intermonitor variation） or two assessments form one measurer （intramonitor variation）was implemented.Results Pearson＇s correlation analysis showed good correlation of bladder pressure measurements between the same and different individuals,and intramonitor and intermonitor correlations coefficients were 0.791 （P＜0.01） and 0.814 （P＜0.01）,respectively.Conclusion With good repeatability,the serf-designed IAP monitoring setting can be accurately and reliably used in critical patients by utilizing a standardized measurement device combined with a standardized clinical protocol.
