喷射呼吸机用于显微喉手术的麻醉时对血液气体的影响

摘    要:[目的 ]研究喷射呼吸机用于显微喉手术的麻醉时对血液气体的影响 .[方法 ]选择美国麻醉学家协会分级Ⅰ~Ⅱ级、拟行择期显微喉手术的病人 3 0例 ,随机分为 2组 ,一组为对照组 ,采用常规气管内插管全身麻醉 ;二组为实验组 ,麻醉诱导后采用喷射呼吸机 ,气管内置入直径 1mm的医用导管 .比较二组病人麻醉前、麻醉中、苏醒时及术后 2h动脉血二氧化碳分压、动脉血氧分压、动脉血氧饱和度以及手术时间、病人苏醒时间、麻醉用药量 .[结果 ]两组病人间各时段血液气体分析相比较无显著差异 ,实验组与对照组比较 ,苏醒时间短、麻醉用药少 .[结论 ]喷射呼吸机用于显微喉手术的麻醉 ,可维持较满意的血液气体水平 ,且与传统方法比较 ,麻醉用药少、病人苏醒时间短 .

Abstract:OBJECTIVE?To study the influence of laryngomicrosurgery by using a jet respirator with general anesthesia on blood gas.?METHODS?30 cases of patients who would receive larygomicrosurgery as Ⅰ~Ⅱ grade of ASA were randomly divided into two groups. The group one was control group , performed by routine general anesthesia, and group two was experimental group ,performed by using a jet respirator with general anesthesia. It was compared that the PaCO 2,PaO 2,SaO 2 of two groups before and after anesthesia , at analepsia ,2hs after surgery, time of surgery and analepsia and dosage of anesthetic.?RESULTS?The blood gas analysis of experimental group had no significant difference as compared with control group, and the experimental group had shorter analepsia time and less dosage of anesthetic.?CONCLUSION? Using a jet respirator with general anesthesia for laryngomicrosurgery provide a satisfying blood gas level, and dosage of anesthetic is less, and the analepsia time is shorter as compared with traditional method of anesthesia.
