11例婴幼儿结核性脑膜炎临床分析

目的:探讨结核性脑膜炎(以下简称结脑)的诊断方法,避免误诊误治。方法:回顾性分析2000年1月至2005年12月在我院诊治的结脑患儿11例的临床资料。结果:本组11例患儿均做过脑脊液(CSF)检查及头颅MRI平扫和增强,结脑临床表现情况多样,传统结核感染指标在婴幼儿不典型;TB-PCR、头颅MRI的检查为早期快速诊断提供依据。结论:综合应用多种诊断方法,包括注重病史和流行病学资料,CSF常规、生化及细胞学动态观察,TB-PCR有较高的特异性、头颅MRI对确诊有较大的帮助。

The Analysis of 11 Clinical Cases of Infants with Tuberculosis Meningitis

Objective: To investigate the diagnostic method of infantile tuberculous meningitis(TBM) and to avoid the high rate of misdiagnosis and wrong therapy. Methods: 11 cases hospitalized infants diagnosed as TBM from Jan ,2000 to Dec, 2005 were retrospectively analyzed. Results:Every case was tested CSF, imaged and contrast enhanced of MRI. and infantile TBM has complicated clinical manifestation. TB- PCR, MRI can provide the basis of early diagnosis. Conclusion: It should be very prudent to diagnose tuberculosis and history of the close contact with patients suffering from tuberculosis, the test about the CSF. The sensitivity and specificity of PCR for diagnosis of TBM was high and MRI can be helpful for the diagnosis of TBM.
