特重型、重型颅脑损伤1500例救治体会

目的:总结1500例特重型、重型颅脑伤病人的救治体会。方法:对我科1990年1月至2002年2月收治的特重型、重型颅脑损伤病人进行回顾性的分析总结。结果:1500例中治愈及好转946例(63.1％),重残104例(6.9％),死亡450例(30％)。结论:尽早清除颅内血肿,大骨瓣减压,脑挫裂伤灶和/或脑叶切除,静脉窦修补重建,及时处理重要脏器损伤,保持呼吸道通畅,维持内环境稳定及防止并发症等措施,是提高救治特重型、重型颅脑损伤病人的成功率,降低致残率及死亡率的关键。

Cure experience of extra- heavy duty, heavy duty craniocerebral injury 1500 cases

Objective To summarize the cure experience of 1500 case extra-heavy duty, heavy duty craniocerebral injury patients. Methods Retrospective analyses and summarize of the extra-heavy duty, heavv duty craniocerebral injury patiente cure in our department from November 1990 to February 2002 were proceeded. Results Cure and straighten 946 cases (63. 1% ) , severe cripplehood 104 cases (6. 9% ), death 450 cases (30% ) in 1500 cases. Conclusion at the earliest opportunity cleanup intracranial hematoma, great pastern bone flap decompression , Conclusion Contusion andlaceration of brain and/or lobe excision , sinus repair and reconstruction , prompting attention to important viscera traumata , keeping respiratory tract fly-over, maintainance of homoiostasis and prevention of complication and others measure, those are key to increase rate of success , reduce rate of remnant and mortality of extra-heavy duty, heavy duty craniocerebral injury patients.
