宫腔镜电切术治疗难治性不全流产

摘    要:目的探讨难治性不全流产的治疗方法。方法回顾性分析本院官腔镜电切术治疗难治性不全流产病例6例。6例患者均有2次以上官腔操作史,曾试用宫腔镜检查定位清除组织物失败。所有患者均有正常月经周期。用电切镜下对残留组织镜下诊断、定位,电切,切除组织全部送病理检查。结果全部病例经官腔镜电切术一次性将残留物完全清除,病理均证实为残留妊娠组织,未出现并发症,术后1个月均恢复正常月经,3例不孕症患者成功受孕。结论宫腔内残留组织电切术可在宫腔镜直视下完全清除陈旧机化组织,恢复宫腔正常形态,对难治性不全流产有重要的治疗意义。

Abstract:Objective To investigate the treatment of intractable incomplete abortion.Meth- ods Case study.6 cases of refractory incomplete abortion treated by transeervical resection under hystero- scope in our hospital from 2003 to 2006 were reviewed.All cases had undergone dilation and curettage more than two times,and had tried curettage with hysteroscope but failed.All cases had normal menstrual cycle. Residual masses were detected,positioned and cut out for pathologic diagnosis.Results Residual tissue was completely and successfully removed without any complications for all 6 patients,and pathologically con- finned as residual gestation tissues.Patient's menstrual cycles were recovered after 1 month,and 3 infertile patients became pregnant later on.Conclusion It's not easy to clean up the residual tissues by repeated dilation and curettage for intractable incomplete abortion.However,transcervical resection is effective in re- moval of organized residual tissues and treatment of intractable incoomplete abortion.
