社区-医院一体化管理模式对2型糖尿病控制的效果评价

【目的】观察社区-医院一体化管理模式对2型糖尿病控制的疗效。【方法】从桂林市南门社区卫生服务中心建档2型糖尿病中随机抽取170例2型糖尿病患者，采取社区-医院一体化管理模式对患者进行管理，对观察对象管理前后的空腹血糖、餐后2h血糖、血压、糖化血红蛋白、低密度脂蛋白、体重指数及尿微量白蛋白指标进行自身对照研究。【结果】经过6个月该模式管理，患者空腹血糖、餐后2h血糖、血压、糖化血红蛋白、低密度脂蛋白、体重指数及尿微量白蛋白等指标均明显改善，达标率均明显升高，有统计学意义（P〈0．01）。【结论】社区-医院一体化管理模式对于2型糖尿病的防治具有重要意义。

Evaluation on the Efficacy of Community/hospital Integration Management Mode for the Control of Type 2 Diabetes Mellitus

[Objective] To observe the efficacy of community/hospital integration management mode for the control of type 2 diabetes mellitus（T2DM）. [Methods] A total of 170 T2DM patients were randomly drawn from the files of Guilin Nanmen community health service center. All patients were managed by community/hospital integration management mode. Fasting blood glucose（FBG）, 2hPBG, HbAlc, systolic blood pressure（SBP）, diastolic blood pressure（DBP）, low-density lipoprotein（LDL）, body mass indcx（BMI） and urine microalbumin（UMA） were studied before and after management by using self-control contrast method. [Results] After management for 6 months, FBG, 2hPBG, HbAlc, SBP, DBP, LDL, BMI and UMA of T2DM patients were improved obviously, and there were significant differences （ P 〈 0.01）. [Conclusion] Community/hospital integration management mode for the management of T2DM patients has more superiority. It is important to play the full role of community/hospital integration management mode for the prevention and cure of T2DM.
