MMP-9、TIMP-1和VEGF在牙龈癌中的表达及意义

【目的】探讨基质金属蛋白酶9（MMP-9）、基质金属蛋白酶抑制剂1（TIMP-1）及血管内皮生长因子（VEGF）在牙龈癌组织中的表达及相关性。【方法】采用免疫组化SP法检测66例牙龈癌和16例正常牙龈组织中MMP-9，TIMP-1和VEGF的表达情况，分析其表达的相关性。【结果】在高分化、中分化及低分化牙龈癌中MMP-9和vEGF的阳性表达差异均有显著性（P〈0．05），而TIMP-1的阳性表达差异无显著性（P〉0．05），MMP-9与TIMP-1在牙龈癌中的表达存在负相关（P〈0．05）；MMP-9与VEGF在牙龈癌中的表达存在正相关（P〈0．05），而TIMP-1与VEGF的表达无显著相关（P〉0．05）。【结论】MMP-9，TIMP-1和VEGF三者可能通过“血管生成”通路参与了牙龈癌的发生和发展过程，联合检测三项指标的表达状况可能有助于牙龈癌的诊断、治疗和评估预后。

Expressions of MMP-9, TIMP-1 and VEGF in Gingival Carcinoma and Their Significance

[Objective]To explore the expressions of matrix metalloprotein-9（MMP-9）, tissue inhibitor of metalloproteinase-l（TIMP-1） and vascular endothelial growth factor（VEGF） in gingival carcinoma and their correlation. [Methods]The expression of MMP-9, TIMP-1 and VEGF in 66 gingival carcinoma tissues and 16 normal gum tissues were detected by SP immunohistochemistry method. The correlation of the expressions with clinical pathology and prognosis was analyzed. [Results] There was significant difference in the positive expression of MMP-9 and VEGF among the well-differentiated, moderately-differentiated and poorly-differen- tiated gingival carcinoma（ P d0.05）, while there was no significant difference in the positive expression of TIMP-1 among 3 differentiated gingival carcinoma（ P 〉0.05）. The expression of MMP-9 was negatively correlated with the expression of TIMP-1 in gingival carcinoma（ P d0.05）, but it was positively correlated with the expression of VEGF（ P〈0.05）. There was no correlation between the expression of TIMP-1 and VEGF （ P 〉0.05）. [Conclusion]MMP-9, TIMP-1 and VEGF may he involved in the pathogenesis and development of gingival qarcinoma through the pathway of angiogenesis. The combination detection of MMP-9, TIMP-1 and VEGF may be helpful for the diagnosis, treatment and prognostic assessment of gingival carcinoma.
