KLK7及E-cadherin在胰腺癌组织中的表达及其临床意义

摘    要:目的探讨组织激肽释放酶7(Kallikrein 7,KLK7)和上皮性钙粘蛋白(E-cadherin,E-cad)在胰腺癌组织中的表达及其临床意义。方法应用免疫组化法检测50例胰腺癌组织、50例癌旁组织及50例正常胰腺组织中KLK7和E-cad的表达,将二者的表达情况与临床病理特征的关系进行统计分析。结果①KLK7在胰腺癌组织中的阳性表达率(72.0%)及表达水平(0.80±0.14)均显著高于癌旁组织及正常胰腺组织(分别为18%、14%和(0.21±0.12)、(0.18±0.10),均p<0.05),而E-cad在胰腺癌组织中的阳性表达率(44.0%)及表达水平(0.51±0.09)均显著低于癌旁组织及正常胰腺组织[分别为100.0%、100.0%和(0.99±0.05)、(1.00±0.02),均p<0.05];②KLK7和E-cad在胰腺癌组织中的表达与患者年龄、组织类型无关;随着临床分期增高、组织分化降低、患者手术时有淋巴结或大网膜转移和腹腔液中有癌细胞,KLK7的阳性表达率呈上升趋势(p<0.05),而E-cad的阳性表达率呈下降趋势(p<0.05或p<0.01);③在胰腺癌中KLK7和E-cad表达呈负相关(p<0.05)。结论 KLK7在胰腺癌组织中呈高表达,而E-cad正好相反,二者与胰腺癌的侵袭转移有关,可为评估预后的有效指标;KLK7和E-cad在胰腺癌组中表达呈负相关,在胰腺癌侵袭转移中可能存在拮抗效应。

Abstract:Objective To study the expression and significance of KLK7 and E -cadherin in pancreatic carcinoma. Methods Immunohistochemistry were performed to investigate the expression of KLK7 and E - cadherin in 50 patients withpancreatic carcinoma, the clinical significance of KLK7 and E - cadherin was analyzed. Results The positivity and relative contents of KLK7 in the patients with pancreatic carcinoma (72.0% ,0. 80±0. 14) were all higher than those in pancreatic carcinoma's surrounding tissue (18% ,0. 21±0. 12) and normal pancreatic tissues(14%, 0. 18 ±0. 10, p 〈 0. 05), but E - cadherin is opposite(44.0% ,100.0% ,100.0% and 0. 51 ±0. 09,0. 99 ±0. 05,1.00 ±0. 02,p〈 0. 05). The positivity and relative contents of KLK7 in the patients with pancreatic carcinoma were correlated with their clinical stage and lymph node metastasis (p 〈 0. 05 ). Moreover there was a negative relationship among E - cadherin and KLK7 (p 〈 0. 05 ). Conclu- sion KLK7 is highly expressed in pancreatic carcinoma, but E - cadherin is opposite. Different expression levels are correl- ative with the occurrence and development of pancreatic carcinoma. They may be effective index of prognosis. The expression of KLK7 and E - cadherine in pancreatic carcinoma is negative correlation. It indicates that they were likely to be antagonistic effect in invasion and metastasis of pancreatic carcinoma.
