经直肠高强度聚焦超声治疗良性前列腺增生的临床分析

摘    要:目的探讨经直肠高强度聚焦超声治疗前列腺增生的有效性和安全性。方法应用Sonab-late 500型"高强度聚焦超声刀"(又称"海伏刀")治疗机治疗前列腺增生患者42例,治疗6个月后定期随访观察患者国际前列腺症状评分(IPSS)、生活质量评分(QOL)、前列腺体积(Pv)、残余尿(RU)及最大尿流率(Qmax)。结果 42例前列腺增生患者治疗后平均随访6个月,IPSS由术前(25.9±4.7)分降到术后(14.5±5.1)分,QOL由术前(4.1±0.8)分降到术后(3.1±1.1)分,Pv由术前(66.8±20.9)ml下降到(50.4±21.7)ml,Qmax由术前(9.4±4.0)ml/s增加到术后(14.4±4.4)ml/s,Ru由术前(145.3±43.3)ml降到术后(58.7±21.9)ml。国际前列腺症状评分(IPSS)、生活质量评分(QOL)、前列腺体积(Pv)、最大尿流率(Qmax)及残余尿(RU)均达到满意的效果。结论经直肠高强度聚焦超声刀治疗前列腺增生安全有效,可以作为一种微创、无痛苦治疗前列腺增生疾病的新方法。

Abstract:Objective To study the curative efficacy and safety of high intensity focused ultrasound (HIFU) in treating benign prostatic hyperplasia(BHP). Methods Sonablate - 500TM was used in this study including 42 ca- ses. International prostate symptom score (IPSS), quality of life index (QOL), prostatic volume (Pv), maximum u- rinary flow rate (Qmax) and remaining urine( RU ) were observed regularly at 6 months after HIFU. Results IPSS decreased from (25.9 ±4. 7) to (14. 5 ±5.1) ,QOL decreased from (4. 1±0. 8) to (3.1 ± 1.1), Pv decreased from (66. 8±20.9) ml to (50. 4 ± 21.7 ) ml, Qmax rose from (9. 4 ± 4. 0) ml/s to ( 14.4 ± 5.4) ml/s, RU decreased from (145.3±43.3)ml to (58.7 ± 21.9)mlo All observing indexes obtained the satisfying results. Conclusion HIFU is not only an effective and safe method but also a Minimally Invasive and painless treatment for BPH.
