子宫切除对内分泌及血脂代谢的影响

目的：了解不同年龄组不同手术方式的女性在子宫切除术后生殖内分泌的变化以及脂代谢的变化。方法：对62例育龄妇女及66例经绝经妇女因良性疾病手术治疗的患者，分别于手术前及手术后2个月测定外周血促卵泡生成素（FSH）、黄体生成素（LH）、雌二醇（E2）、孕酮（P）、睾酮（T）、甘油三酯（TG），胆固醇总量（TC）、高密度脂蛋白（HDL）、低密度脂蛋白（LDL）、载脂蛋白A1（ApoA1）和载脂蛋白B（ApoB）。结果：育龄妇女手术后性激素FSH、LH、T均有升高（P＜0．01），E2、P下降（P＜0．01），血中TG、LDL,ApoB浓度升高（P＜0．05－P＜0．01），HDL，ApoA1明显下降（P＜0．01）。绝经后妇女手术前后FSH和LH升高，而E2和P降低（P＜0．01）；手术后LDL明显增高，ApoA1下降（P＜0．01）。结论：子宫切除术后雌激素明显下降，血脂明显升高，但不同年龄组妇女性激素变化及血脂代谢不同。

Changes of endocrine and serum lipid metabolism in women after hysterectomy

Objective:To explore the changes of hormone level and serum lipid metabolism in femal patients of different age groups who had received hysterectomy with different operation methods.Methods:Sixty two patients of childbearing age and 66 patients of menopause with gynocologic benign diseases were performed hysterectomy.The level of follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E 2),progesterone(P),testosterone(T),triglycerine(TG),total cholesterol(TC),high density lipoprotein(HDL),low density lipoprotein(LDL),apoprotein A 1(ApoA 1) and apoprotein B(ApoB),were measured before the operation and two months after the operation.Results:In women of childbearing age,the level of FSH,LH and T increased( P <0.01) while that of E 2 and P declined( P <0.01).The level of TG,LDL,ApoB increased( P < 0.05 to P <0.01),while that of HDL and ApoA 1 decreased significantly( P <0.01).In menopaused women the level of FSH and LH increased,while that of E\-2 and P declined( P <0.01) before and after the operation.The level of LDL increased significantly and that of ApoA 1 decreased( P <0.01) after the operation.Conclusions:After hysterectomy,the estrogen decline obviously while the serum lipid decrease significantly,but the changes are different in women of different ages.
