剖宫产术后腹壁切口子宫内膜异位症68例临床分析

目的探讨剖宫产术后腹壁切口子宫内膜异位症的病因、治疗方法及预防。方法 2005年6月-2010年6月收治的剖宫产术后腹壁切口子宫内膜异位症68例,按治疗方法将患者分为手术组（44例）和药物组1（12例）、药物组2（12例）,观察三组的治疗效果及一年后的复发率,并进行分析。结果 44例手术治疗的患者治愈44例（治愈率：100.00%）,复发2例（复发率：4.54%）;药物组1治愈6例（治愈率：50%）,复发4例（复发率：66.67%）;药物组2治愈7例（治愈率：58.33%）,复发5例（复发率：71.42%）。手术组治愈率明显高于药物组治愈率,复发率明显低于药物组,差异显著,具有统计学意义（P〈0.05）。结论手术切除异位病灶是治疗腹壁切口子宫内膜异位症的首选方法,术中避免子宫内膜组织的种植是预防该病的关键。

The Clinical Analysis of 68 Endometriosis Cases on Abdominal Incision After Cesarean Section

Objective To investigate the cause,remedy and prevention of endometriosis on abdominal incision after Cesarean Section. Methods Review of 68 endometriosis examples after Cesarean Section received and cured in our hospital from June 2005 to June 2010. Patients were divided into three groups as different treatment,operation group（ 44examples） 、drug group 1（ 12 examples） and drug group 2（ 12 examples）,to investigate the curative effect and recurrence rate and carry out the analysis. Results 44 examples recovered in operation group（ recovery rate ： 100%）,2examples recurred（ recurrence rate： 4. 54%）; 6 examples recovered in drug group 1（ recovery rate： 50%）,4 examples recurred（ recurrence rate： 66. 67%）; 7examples recovered in drug group 2（ recovery rate： 58. 33%,5examples recurred（ recurrence rate ：71.42%）. The recovery rate of operation group is apparently higher than that of drug group,the recurrence rate of operation group is apparently lower than that of drug group,. The difference is significant（ P〈 0.05）.Conclusions Surgical removal of ectopic lesions is the preferred method to treat abdominal incision endometriosis,avoiding endometrial tissue cultivation in operation is the key to prevent this disease.
