耳内镜下鼓室置管治疗鼻咽癌放疗后并发分泌性中耳炎的研究

摘    要:目的:探讨耳内镜下鼓室置管治疗鼻咽癌(NPC)放疗后并发分泌性中耳炎(SOM)的临床疗效和最佳置管时间。方法:应用耳内镜下鼓室置管术治疗鼻咽癌放疗后并发分泌性中耳炎患者52例(87耳),分为A组(置管时间<6个月)17例(28耳),B组(置管时间6~12个月)18例(30耳),C组(置管时间>1年)17例(29耳)。结果:全部病例置管均一次完成,治疗总有效率为75.8%(66/87耳),B组的有效率为90.0%,明显高于A组和C组(P<0.05);B组的置管术后复发率低,主要并发症发生率少(P<0.05)。结论:耳内镜下鼓室置管治疗鼻咽癌放疗后并发SOM安全有效,置管时间以6~12个月为宜。

Endoscopic tube Insertion in treatment of secretory otitis media after following radiotherapy for nasopharynge-al carcinoma

Abstract:Objective To explore the clinical curative effect and the optimal indwelling time of the endoscopic tube Insertion in treatment of nasopharyngeal carcinoma (NPC) complicated with secretory otitis media after radiotherapy (SOM). Method Endoscopic tube Insertion in treatment of patients with SOM after radiotherapy NPC patients in 52 eases (87 ears) ,group A in 17 cases with 28 ears,the indwelling time lower than 6 months,group B in 18 cases with 30 ears,the indwelling time from 6 months to one year,group C in 17 eases with 29 ears, the indwelling time longer than 6 months. Results All cases of catheter was completed, the total effective rate was 77% (67/ 87 ears), the rate in group B was 90% ,it was significantly higher than that in A and C group ( P 〈 0. 05 ), the rate of recurrence after operation and the main complications in B group was lower than other groups(P 〈 0.05). Conclusion Tube Insertion in the treatment of nasopharyngeal carcinoma after radiotherapy concurrent SOM is effective, and the indwelling time from 6 to 12 months is appropriate.
