不稳定型心绞痛直接冠状动脉内支架置入术的临床观察

目的：探讨直接冠脉内支架置入术治疗不稳定型心绞痛的可行性和安全性。方法：回顾性分析124例不稳定型心绞痛患者支架置入术的临床资料及影像学资料，比较直接支架置入术与预扩张后支架置入术患者血管的病变特点。结果：124例患者86例尝试直接支架置入术，79例成功，成功率为91．9％；其中A型病变64例，B1型病变25例，B2型病变3例，无C型病变；分叉病变11例，开口病变9例，无闭塞病变；长病变（≥10mm)44例。7例失败者，5例血管严重成角，2例钙化病变。结论：直接支架置入术在有选择的不稳定型心绞痛患者中具有满意的可行性及安全性。

CLINICAL INVESTIGATION OF THE EFFICACY AND SAFETY OF DIRECT CORONARY STENT IMPLANTATION IN UNSTABLE ANGINA PECTORIS

Objective:To evaluate the feasibility and safety of direct coronary stent implantation in unstable angina pectoris.Methods:The clinical and angiographic data of 124 unstable angina pertoris patients with coronary stent implantation were retrospectively reviewed comparing lesion characteristics of direct stent implantation( n =86) with those of the standard coronary stent implantation technique.Results:In 86 patients,direct stenting was attempted and successfully deployed in 79 patients(91 9%).In direct stent group,7 patients failed to implant the stent directly(5 patients had a very tortuous coronary and 2 had moderate vessel calcification).Lesions in the direct senting group were predominantly type A( n =62) and type B( n =19).Conclusion:Direct coronary stent implantation is feasibile and safe in selected unstable angina pectoris patients.
