64排螺旋CT不同浓度对比剂在颈部血管成像中的应用价值

摘    要:目的 探讨64排螺旋CT低浓度对比剂在颈部血管成像(CTA)扫描中的应用价值。方法 选取2014年1月至2015年11月柳州市人民医院的108例颈部CTA检查患者,随机分成A、B、C组,每组36例。A组给予高浓度对比剂碘普罗胺(370 mgI/mL),120 kV;B、C组分别给予低浓度对比剂碘佛醇(320 mgI/mL)、碘海醇(300 mgI/mL),100 kV,3组均给予50 mL对比剂,根据原始图像及重组图像进行综合性分析。结果 所有受检者均顺利完成检查。3组病例的主动脉、颈总动脉、基底动脉、舌动脉均清晰显示,动脉造影剂浓度维持在较高水平,A组主动脉CT值(338.40±78.80)HU,颈总动脉CT值(392.85±74.03)HU,基底动脉CT值(311.77±55.11)HU,舌动脉CT值(320.05±54.32)HU;B组主动脉CT值(297.15±67.59)HU,颈总动脉CT值(363.40±71.93)HU,基底动脉CT值(280.25±43.46)HU,舌动脉CT值(292.12±46.33)HU;C组主动脉CT值(280.19±66.77)HU,颈总动脉CT值(351.64±75.38)HU,基底动脉CT值(277.41±45.22)HU,舌动脉CT值(287.35±49.35)HU。A组CT值稍高于B、C组,但差异无统计学意义(P>0.05);重组图像质量评分,A组为(4.53±0.38)分,B组(4.41±0.34)分,C组(4.42±0.29)分,3组评分比较差异无统计学意义(P>0.05)。结论 64排螺旋CT颈部CTA使用低浓度对比剂可获得较好的图像质量,可满足临床需要。

Abstract:Objective To explore the application value of low concentration contrast agent in the neck 64-slice spiral CT angiography (CTA).Methods From Jan 2014 to Nov 2015, 108 patients undergoing neck CTA in our hospital were randomly divided into three groups of A, B and C, each group of 36 cases, and were administered with 50 ml contrast agent. Among them, group A were given high concentration contrast agent of ultravist (370 mgl/ml, 120 kV), and group B and C were given low concentration contrast agent of ioversol or iohexol (320 mgl/ml vs 300 mgl/ml, 100 kV), respectively. Their original and reconstruction images were comprehensively analyzed.Results All subjects were examined successfully, and their aorta, cephalic artery, base artery and lingual artery were displayed clearly, with arterial contrast medium at higher levels. The CT values of group A were (338.40±78.80) HU at aorta, (392.85±74.03) HU at cephalic artery, (311.77±55.11) HU at base artery and (320.05±54.32) HU at lingual artery. The CT values of group B at the aorta, cephalic artery, base artery and lingual artery were (297.15±67.59) HU, (363.40±71.93) HU, (280.25±43.46) HU and (292.12±46.33) HU, respectively. The CT values of group C were (280.19±66.77) HU, (351.64±75.38) HU, (277.41±45.22) HU and (287.35±49.35) HU, respectively. Although the CT values in the A group were slightly higher than those in the B and C group, but there wasn''t statistically significant difference among them (P>0.05). The scores of reconstruction image quality in the three groups were (4.53±0.38), (4.41±0.34) and (4.42±0.29), respectively, and also no significant difference was found (P>0.05).Conclusion The application of neck 64-slice spiral CTA with low concentration contrast agent could obtain better image quality meeting clinical needs.
