超声心动图对新生儿肺动脉高压的诊断价值

摘    要:目的 探讨超声心动图对新生儿肺动脉高压的诊断价值。方法 收集2011年6月至2014年12月接收的80例新生儿,其中经过超声心动图证实患有新生儿持续肺动脉高压(PPHN)的新生儿38例,根据肺动脉压力分为轻度组(20例)、中度组(12例)、重度组(6例),其余42例新生儿为对照组。通过超声心动图测量比较各组患儿心脏各腔室大小,并对比分析测量结果。结果 患儿心脏右房内径、右室内径及主肺动脉内径随着肺动脉收缩压(PASP)的增高均有增大趋势。对照组与轻度组、轻度组与中度组、中度组与重度组间右心房内径(RA)、右心室内径(RV)、主肺动脉内径(MPA)分别作两组样本t检验,结果差异均有统计学意义(P<0.05);随着PASP的增高,新生儿低氧血症(PO2)逐渐加重,各组样本间差异有统计学意义(P<0.05)。结论 超声心动图可对新生儿肺动脉高压进行准确诊断,对判定患儿预后、临床诊疗均有重要价值。

Abstract:Objective To explore the value of echocardiography in the diagnosis of neonatal pulmonary arterial hypertension. Methods From Jun 2011 to Dec 2014, 80 cases of newborns were collected, in which 38 cases with persistent pulmonary hypertension of the newborn(PPHN) were confirmed by echocardiography, and divided into the mild group(20 cases), the medium group(12 cases) and the severe group(6 cases) according to their pulmonary artery pressure. The rest 42 cases were included in the control group. The heart chamber size of newborns in each group was measured by echocardiography, and the results were compared and analyzed. Results The right atrial diameter, right ventricular diameter and main pulmonary artery diameter of children increased with the pulmonary artery systolic pressure(PASP). Between the control group and the mild group, the mild group and the medium group, the medium group and the severe group, the paired sample t test in the diameter of right atrial(RA), right ventricular(RV) and main pulmonary artery(MPA) all showed statistically significant difference(P<0.05). With the increase of PASP, the neonatal hypoxemia gradually aggravated, and the differences between group samples also were significant(P<0.05). Conclusion Echocardiography could accurately diagnose the pulmonary artery hypertension of newborns and is valuable to determine their prognosis and clinical diagnosis and treatment.
