不同年龄组下消化道出血患者结肠镜检查结果分析

目的研究不同年龄组下消化道出血的病因特点。方法对175例年龄>60岁(老年组)和225例年龄为18~59岁(中青年组)以便血为主诉的就诊者,采用电子结肠镜检查。结果导致下消化道出血的前4种疾病是:老年组为大肠癌56例(14.0%)、大肠息肉32例(8.0%)、痔23例(5.75%)、溃疡性结肠炎16例(4.0%),中青年组为溃疡性结肠炎45例(11.3%)、大肠息肉37例(9.25%)、结肠炎(不明原因者)35例(8.75%)、痔30例(7.5%)。并且疾病多位于乙状结肠和直肠,约占63%。结论大肠癌、大肠息肉、溃疡性结肠炎、痔为两组下消化道出血的常见病因。大肠癌居老年组首位,溃疡性结肠炎居中青年组首位。结肠镜检对下消化道出血的诊断有重要的价值。

Analysis of the Coloscopy Results in Various Age Group Patients with Lower Alimentary Tract Hemorrhage

Objective To investigate the causes of lower alimentary tract hemorrhage in various age groups.Methods The clients principally present hematochezia,including 175 cases whose age exceed 60 years old(older age group) and 225 cases whose age exceed 18 years but less than 59 years old(middle-young people group),adopt electronic colonoscopy.Results The most common causes resulted in lower alimentary tract hemorrhage in old people were four disease,as follow: carcinoma of large intestine(56 cases,14.0%),large intestinal polyp(32 cases 8.0%),piles(23 cases,5.75%),ulcerative colitis(16 cases,4.0%).The diseases resulted in hematochezia in middle-young people were ulcerative colitis(45 cases,11.3%),colorectal polyp(37 cases,9.25%),primary colitis(35 cases,8.75%) and piles(30 cases,7.5%).The most involved regions were colon sigmoideum and rectum.Conclusions Cancer of colon,colorectal polyp,ulcerative colitis and piles were common causes of lower alimentary tract hemorrhage in old,middle and young people.Cancer of colon was the first cause in old people;however,ulcerative colitis was the first cause in middle-young people.Therefore,digital rectal examination and colonoscopy play important roles in the diagnosis of lower alimentary tract hemorrhage.
