摘    要:目的研究胰岛素样生长因子-2(IGF-2)在冠状动脉粥样硬化性心脏病中的表达及其临床意义。方法以放射免疫法分别测定59例冠心病患者(合并高血压24例,血压正常者35例)和49例非冠心病对照组(合并高血压21例,血压正常者28例)血清IGF-2的水平。结果血清IGF-2水平在冠心病合并高血压组、冠心病血压正常组、高血压组、正常对照组之间差异统计学意义(P〈0.05);排除高血压因素后,IGF-2水平在正常对照组明显低于稳定性心绞痛组和急性冠脉综合症组(P〈0.05),而冠心病两亚组之间差异无统计学意义(P〉0.05);血清IGF-2水平与冠状动脉狭窄评分之间存在正相关关系。结论高血压及冠心病均是影响血清IGF-2水平的重要因素,且两因素间存在交互作用;血清IGF-2水平的高低可作为判断冠脉狭窄程度的指标,但不能作为判断急性冠脉综合征的判断指标。

Abstract:Objective To study the expression and clinic significance of insulin-like growth factor-2( IGF-2 ) in coronary artery disease. Methods Radioimmunity was used to determine respectively the blood serum level of IGF-2 in 59 patients (24 combining hypertension and 35 without hypertension) with coronary artery disease and 49 patients( 21 combining hypertension and 28 without hypertension) without coronary artery disease. Results The blood serum level of IGF-2 has significant difference among the coronary artery disease merging hypertension group, simple hypertension group, simple coronary artery disease group and norreal control group (P 〈 0.05 ). After eliminating the hypertension factor, the blood serum level of IGF-2 in control group are obviously lower than in stability angina group and acute coronary syndrome group( P 〈 0.05 ) , however there is no significant difference between the two CAD subgroups(P 〉0.05 ). There exist a positive correlation relation between the growth factor and coronary stenosis scores. Conclusion Hypertension and coronary artery disease all can effect the growth factor level, furthermore, there exist interaction;The blood serum level of IGF-2 can be used to judge the stegnosis degree of coronary artery, but it can not be used to determine the acute coronary syndrome.
