不同时期放置宫内节育器的临床效果及副反应初步观察

本文以前瞻性方法研究不同时期放置宫内节育器（ＩＵＤ）２５０例，其中产后即时１５６例，剖宫产后即时６０例，产后３０～９０天３２例及中期妊娠引产后２例。并选择同期年龄、胎产次、产程和产科情况相似的未放置ＩＵＤ者１００例产妇作为对照，进行临床效果、副反应观察，并随访一年。结果表明：不同时期放置ＩＵＤ无一例发生产科并发症和感染；产后出血发生率、产后恶露天数、各组与对照组间无显著性差异（Ｐ＞０．０５），哺乳量及月经恢复时间也基本相似，续用率在产后即时组一年为９３．６％，剖宫产后即时组９８．３％，产后３０～９０天组９６．９％；副反应发生率３个月时为２０％，６个月时１７．５％，１２月时１５％，有逐渐下降趋势，且经对症处理后无一例因症取出ＩＵＤ者。本研究提示：以上四个时期放置ＩＵＤ是安全、可行、可靠的避孕方法。

Preliminary Observation of Clinical Efficacy and Adverse Reaction after put IUD during different postpartum stages

A prospective study of 250 cases put IUD during different postpantum stages was reported,including 156 cases immediate after birth,60 cases during Caesarean section,32 cases 30 to 90 days after birth,and two cases immediatly after induced labour of midtrimester pregnancy.one humdred postpartum women who had comparable age,parity and obstetric condition but had not been put IUD in the same period were taken as control group to study their clinieal efficacy and adverse reaction.Both geoups were followed up to one year.Results showed: (1) No infection and other obstetric complica-tions occured among all cases putting IUD during different stages;(2) Percentage of hemorrhage,amount of lochia of various IUD putting groups have no significant difference compared with control group.(3) Lactation period and menses recovery were also alike in various groups;(4) Adverse reactions of IUD putting group are fewer than traditional methods.all of them IUD were not taken out after symptoms had been treated.So application of IUD in reported postpartum periods are safe,appropriate and accptable.
