FoxM1剪接异构体在上皮性卵巢癌中的表达及临床意义

摘    要:目的:观察 FoxM1剪接异构体在女性上皮性卵巢癌中的表达特征,并探讨其应用价值。方法恶性组50例,为经手术治疗及术中获取病灶组织病理检测确诊的上皮性卵巢癌患者;良性组50例,为影像学及病理诊断为卵巢良性肿瘤者;正常组50例,为其他妇科疾病入院但经临床症状、影像学检查等未见卵巢异常者。均取卵巢病理组织切片免疫组化染色,免疫荧光法检测 FoxM1表达阳性情况。结果恶性、良性和正常组卵巢组织 FoxM1的表达阳性率比较差异有统计学意义(χ2=21.141,P ﹤0.001),两两比较,恶性组显著高于良性组及正常组(χ2=16.284、5.363,P ﹤0.01),良性组显著高于正常组(χ2=10.187,P ﹤0.01);统计结果显示 FoxM1阳性表达与上皮性卵巢癌FIGO 分期、分化程度以及淋巴结转移密切相关,差异有统计学意义( P ﹤0.05或﹤0.01);经单因素、多因素分析显示,只有 FIGO 分期、淋巴结转移可作为上皮性卵巢癌患者的独立预后因素。结论 FoxM1剪接异构体在上皮性卵巢癌中呈现为高表达,且与上皮性卵巢癌 FIGO 分期、分化程度、病理分型以及淋巴结转移密切相关;经分析,上皮性卵巢癌患者的独立预后因素为 FIGO 分期、淋巴结转移和 FoxM1表达阳性。

Expression and clinical significance of FoxM1 isoforms in epithelial ovarian cancer
Abstract:Objective To observe the expression characteristics of FoxM1 isoform in patients with epithelial ovarian cancer,and to explore its application value. Methods Fifty patients with epithelial ovarian cancer identified by pathological examination who underwent surgical therapy were regarded as malignant group,the other 50 patients with ovarian benign tumor who were identified by image and pathologic examination were served as benign group,besides,50 patients with the other gynecological diseases who had no ovary abnormality identified by image examination and clinical symptoms were regarded as control group. The ovarian pathological tissue sections were made and stained by immunohistochemistry,then the positive expression rates of FoxM1 were detected by immunofluorescence. Results There were significant differences in the positive expression rates of FoxM1 among malignant group,benign group and control group( P ﹤ 0. 01),which in malignant group were significantly higher than those in benign group and control group( P ﹤ 0. 01),moreover,which in benign group were significantly higher than those in control group( P ﹤ 0. 01). The positive expression rates of FoxM1 were closely correlated to FIGO staging,differentiation degree and lymph node metastasis of epithelial ovarian cancer( P ﹤ 0. 05 or P ﹤ 0. 01). The single factor and multiple factors analysis showed that only FIGO staging and lymph node metastasis could be regarded as independent prognostic factors in patients with epithelial ovarian cancer. Conclusion FoxM1 isoforms appear overexpression in epithelial ovarian cancer,furthermore,which are closely correlated to FIGO staging,differentiation degree,pathological typing and lymph node metastasis of epithelial ovarian cancer. The independent prognostic factors of epithelial ovarian cancer are FIGO staging,lymph node metastasis and positive expression of FoxM1.
