散光型人工晶状体治疗合并角膜散光的白内障临床疗效评价

目的：评价散光型人工晶状体（Toric IOL）治疗合并角膜散光的白内障临床疗效。方法：将白内障患者100例（143眼）随机分为两组，对照组（n ＝50，72眼）行超声乳化白内障摘出术＋球面 IOL植入术，观察组（n ＝50，71眼）行超声乳化白内障摘出术＋Toric IOL 植入术；比较两组患者的治疗效果和随访结果。结果：两组患者术后的裸眼视力均较术前有改善，差异具有统计学意义（P＜0．05），但观察组的改善幅度优于对照组，差异具有统计学意义（P＜0．05）；观察组的总和散光优于对照组，差异具有统计学意义（P＜0．05）；观察组术后的传递函数截止频率值（MTF cut off）、100％对比度下视力、20％对比度下视力改善幅度优于对照组，差异具有统计学意义（P＜0．05），两组患者术后的9％对比度下视力改善幅度比较无差异，差异无统计学意义（P＞0．05）；两组患者末次随访的 IOL 柱镜轴位比较差异无统计学意义（P＞0．05）。结论：Toric IOL 治疗合并角膜散光的白内障患者疗效确切，能有效地矫正散光，视觉质量改善幅度较大，且具有良好稳定性，推广前景光明。

Evaluation of the Clinical Efficacy on Toric IOL in the Treatment of Cataract with Corneal Astigmatism

Objective: To evaluate the clinical efficacy on toric IOL in the treatment of cataract with corneal astigmatism.Methods: 100 patients (143 eyes) with cataract were divided into two groups randomly . The control group (n=50, 72 eyes) was treated with phacoemulsification +spherical IOL implantation.The ob-servation group (n =50, 71 eyes) was treated with phacoemulsification +toric IOL implantation.The treatment effect and follow-up results of two groups was compared .Results:The uncorrected vision acuity of two groups improved evidently after operation(P<0.05), but the improvement degree of the observation group was better than that of the control group (P<0.05).The total astigmatism of the observation group was better than that of the control group(P <0.05).MTF cut off, 100% contrast vision and 20% contrast vision of the observation group after operation improved better than that of the control group (P<0.05).There was no evident difference for the improvement of 9% contrast vision between two groups (P>0.05).There was no evident difference for IOL lenticular axial view for the last follow up between two groups (P>0.05).Conclusion: The patients with cataract with corneal astigmatism treated by Toric IOL not only has evident efficacy and effectively correct a -stigmatism, but also can improve visual quality evidently and is stable , which is worthy of promotion.
