短期胰岛素强化治疗对2型糖尿病患者血糖、血脂及胰岛功能的影响

目的：探讨短期胰岛素强化治疗对初诊2型糖尿病患者血糖、血脂及胰岛功能的影响。方法88例初诊2型糖尿病患者随机分为对照组和观察组，每组44例。对照组行常规口服降糖药治疗，观察组在对照组的基础上进行短期胰岛素治疗。比较2组治疗前、后血糖、血脂及胰岛功能变化情况。结果2组患者空腹血糖（ FPG）、餐后2 h血糖（2 h PG）、糖化血红蛋白（ HbA1c）较治疗前均显著降低，且观察组降低更显著，差异有统计学意义（ P ＜0构．05）；2组患者三酰甘油（ TG）、总胆固醇（ TC）、低密度脂蛋白胆固醇（ LDL-C）较治疗前均显著下降，高密度脂蛋白胆固醇（HDL-C）较治疗前显著升高，观察组各指标差异更显著，差异有统计学意义（ P ＜0．05）；2组患者胰岛素抵抗指数（HOMA-IR）较治疗前显著下降，胰岛素分泌指数（HOMA-β）和空腹胰岛素（FINS）均显著升高，且观察组差异更显著，差异均有统计学意义（ P ＜0．05）。结论短期胰岛素强化治疗可明显改善患者血糖、血脂及胰岛功能。

Effect of short-term intensive insulin treatment on blood glucose,blood lipid and islet function in patients with type 2 diabetes mellitus

Objective To investigate the effect of short-term intensive insulin treatment on glucose , lipid and islet function in patients with type 2 diabetes mellitus .Methods A total of 88 patients with type 2 diabetes mellitus were randomly divided into observation group ( n =44) and control group ( n =44).The Patients in both groups were treated by conventional oral hypoglycemic drugs ,however,the patients in observation group ,on the basis of control group ,were treated by short-term intensive insulin .The levels of blood glucose , blood lipid and islet function before and after treatment were detected and compared between two groups .Results After treatment,the levels of FPG,2 h PG,HbA1c in both groups were obviously decreased,especially in observation group ( P <0.05).After treatment,the levels of TG,TC,LDL-C in both groups were obviously decreased , however , the levels of HDL-C were obviously increased , as compared with those before treatment , moreover,the differences in observation group were more significant than those in control group ( P <0.05).After treatment, the HOMA-IR in both groups was obviously decreased ,however, HOMA-βand FINS were obviously increased ,as compared with those before treatment, especially in observation group ( P <0.05).Conclusion The short-term intensive insulin treatment can obviously improve blood glucose ,blood lipid and islet function of patients with type 2 diabetes mellitus .
