血液超滤治疗在肾功能不全合并急性左心衰竭患者抢救中的作用

【目的】探讨血液超滤治疗在抢救肾功能不全合并急性左心衰竭中的作用。【方法】将40例急性左心衰竭患者根据血肌酐值分为药物组和超滤组，所有患者人院时均监测心率、呼吸、血压、中心静脉压，检测肾功能、动脉血气。入院24h内行心脏彩超检查，并记录患者症状缓解时间。【结果】基线超滤组收缩压、舒张压和中心静脉压明显高于药物组（P〈0．05），PaO2超滤组明显低于药物组（P〈0．05）。症状缓解时心脏彩超显示超滤组LVESD、LVEDD明显高于药物组（P〈0．01），而EF明显低于药物组（P〈0．05），超滤组症状缓解时间明显少于药物组（P〈0．05）。【结论】床旁血液超滤治疗在抢救肾功能不全合并急性左心衰竭患者中有效。

Effects of bedside hemo- ultrafiltration on the renal inadequacy affiliated heart disease patients

Objective] To investigate the effects of bedside hemo - uhrafihration on renal inadequacy affiliated heart disease patients. Methods] According to treat strategy, 40 renal inadequacy patients affiliated heart disease were assigned to conventional therapy group and uhrafiltration therapy group. All the patients were monitored heart rate, breath, blood pressure and central venous pressure whole detected arterial blood gas. Ultrasound was detected in 24 hours and prior to discharge. Results] The patients＇ systolic blood pressure, diastolic blood pressure, central venous pressure in uhrafiltration therapy group are higher than conventional therapy group, but PaO2 was lower in baseline. When symptomatic rehef, LVESD and LVEDD are higher in ultrafiltration therapy group and EF was lower in conventional therapy group. The time of symptomatic relief in ultrafdtration therapy group was shorter than conventional therapy group. Conclusions] Bedside hemo - ultrafiltration had effects on the renal inadequacy affiliated heart disease patients.
