慢性乙型肝炎患者血清共价闭合环状DNA检测的临床意义

目的探讨慢性乙型肝炎(CHB)患者血清中共价闭合环状DNA(cccDNA)的临床检测意义。方法随机选取进行肝组织活检的CHB患者57例。用实时荧光定量PCR法分别检测入选病例血清中cccDNA值,同时定量检测血清中HBV DNA水平,对比HBeAg阳性组和HbeAg阴性组cccDNA水平,并分析cccDNA与HBV DNA、病理炎症分级、ALT的关系。结果 HBeAg阳性组血清中cccDNA水平明显高于HBeAg阴性组(P<0.05);HBeAg阳性组血清中cccDNA与HBV DNA存在正相关关系(P<0.01);cccDNA水平和肝组织炎症活动度及ALT水平比较均无显著性差异(P均>0.05)。结论血清中HBV cccDNA定量是评价HBV复制最客观可靠的指标,在CHB诊断及抗病毒治疗中具有重要意义。

Clinical significance of covalent closed circle DNA in serum detected for patients with chronic hepatitis B

Objective It is to approach the clinical significance of detection of covalent closed circle DNA(cccDNA) in serum of patients with chronic hepatitis B.Methods 57 patients with chronic hepatitis B were randomly selected for liver biopsy.The serum level of cccDNA was detected with real-time fluorescence quantitative PCR method.The serum level of HBV DNA was detected at the same time.The levels of cccDNA in between HBeAg-positive group and HBeAg-negative group were contrasted.The relations between the cccDNA level with the HBV DNA quantitative,inflammation grade and ALT level were analyzed.Results The serum levels of cccDNA in HBeAg-positive group was significantly higher than that in HBeAg-negative group(P<0.05).The serum levels of cccDNA in HBeAg-positive group has positive correlation with the serum level of HBV DNA(P<0.01).There were no significant difference between the serum levels of cccDNA and inflammation activity in hepatic tissue and ALT levels(both P>0.05).Conclusion HBV cccDNA quantitate in serum is the most objective and reliable indicator to evaluate copy of HBV DNA,and has important significance in CHB diagnosis and antiviral therapy.
