动脉血乳酸水平与感染性休克患者的预后关系分析

目的：探讨动脉血乳酸水平与感染性休克患者的预后关系。方法选择2012年1月至2012年6月收入RICU的49例感染性休克患者，按患者预后分为存活组和死亡组。测定入住RICU后0、6、24、48 h动脉血乳酸水平，分析比较两组患者动脉血乳酸检测结果。结果两组患者入住RICU时，动脉血乳酸水平均高于正常范围，随治疗开始乳酸水平逐渐降低，存活组患者入院后0、6、24、48 h动脉血乳酸水平显著低于死亡组，存活组6 h乳酸清除率显著高于死亡组。结论动脉血乳酸水平较高的感染性休克患者预后差，入院ICU后6 h乳酸清除率较低的患者预后较差。

Analysis of the relationship between concentration of arterial blood lactic acid and prognosis of patients with septic shock

Objective To study the relationship between concentration of arterial blood lactic acid （ BLA）and prognosis of patients with septic shock in respiratory intensive care unit（ RICU）. Methods From January 2012 to June 2012,49 patients with septic shock were divided into survival group and death group according to the prognosis. The levels of arterial BLA and results of blood gas analysis were compared between the two groups. Results The levels of arterial BLA in the two groups after being admitted into RICU were significantly higher than the normal range and gradually decreased with continuation of treatment. The levels of arterial BLA in patients of survival group at 0,6,24 and 48 h after being admitted into RICU were distinctly lower than those in death group. The 6 h lactate clearance of patients in survival group was remarkably higher than that in death group. Conclusions The septic shock patients with higher arterial blood lactic acid often has poor prognosis,and the prognosis of patients with low lactate clearance at 6 h after being admitted into ICU is relatively worse.
