霉菌性食管炎78例临床分析

目的：探讨霉菌性食管炎患者的临床发病特点及发病诱因。方法随机选取安阳市人民医院2010年2月至2013年4月收治的诊断为霉菌性食管炎患者78例，对患者的临床病史资料进行回顾性分析。结果78例患者中包括21例重度感染，39例中度感染及18例轻度感染；患者均合并程度不同的肠胃疾病：24例合并胃炎，12例合并消化性溃疡，12例合并胃炎伴胃溃疡，9例合并反流性食管炎，6例合并食管静脉曲张，9例合并胃癌。采用制霉菌素或氟康唑对患者进行治疗，痊愈69例，总有效率为88.5%。结论近年来霉菌性食管炎在临床上的发病率呈逐年上升趋势，在对患者进行治疗时，应尽量避免使用强抑酸剂或抗生素，同时对患者合并的其他胃肠疾病进行积极治疗，避免造成霉菌进一步感染。

Clinical analysis of 78 cases of fungal esophagitis

Objective To investigate the clinical characteristics and predisposing factors of fun-gal esophagitis disease. Methods Seventy-eight patients diagnosed as fungal esophagitis were selected from February 2010 to April 2013 in people’s hospital of Anyang,the patient’s clinical data were retro-spectively analyzed. Results Among the seventy-eight patients,there were 21 cases of severe infection, 39 cases of moderate infection and 18 cases of mild infection. All of the patients were combined with dif-ferent degrees of gastrointestinal disease：24 cases with gastritis,12 cases with peptic ulcer,12 cases with gastritis and ulcers,9 patients with reflux esophagitis,6 cases with esophageal varices,9 cases with stom-ach cancer. All of the patient received nystatin or fluconazole treatment,69 cases cured,the total effective rate was 88. 5% . Conclusions In recent years,the incidence of fungal esophagitis in clinical showed an increasing trend. In the treatment of patients,we should avoid using strong antacids or antibiotics,and give aggressive treatment for patients with other gastrointestinal diseases to avoid further fungal infection.
