股前侧Y形切口双钢板内固定治疗AO-C3型股骨远端骨折

目的探讨股前侧Y形切口双钢板内固定治疗AO分型中C3型股骨远端骨折的可行性及临床疗效。方法自2009-10—2011-08诊治AO分型中C3型股骨远端骨折7例,经股前侧Y形切口入路,直视下复位,以双侧钢板（外侧解剖锁定钢板结合内侧支撑钢板）内固定。结果所有患者均获得平均18.3（12-24）个月随访,术后切口均一期愈合,骨折愈合时间平均18.9（12-28）周,无骨折畸形愈合或不愈合。末次随访时根据Schatzker-Lambert膝关节功能评分评定疗效：优2例,良4例,可1例。结论对于AO分型中C3型股骨远端骨折采用Y形切口双钢板内固定治疗具有关节面显露清晰、骨折解剖复位、内固定强度高等优点。

Treatment of AO-type C3 distal femoral fractures with double-plating fixation via a Y-shaped surgical approach

Objective To investigate the practicability and clinical efficiency of double-plating fixation via a Y-shaped anterior femoral incision in treating AO-type C3 distal femoral fractures. Methods From October 2009 to August 2011,7 cases of AO-type C3 closed distal femoral fractures were treated in our department. Operations were taken after hospitalization via a Y-shaped anterior femoral incision, the fractures were repositioned directly and fixed with double plates（a lateral anatomical locking plate combined with a medial buttress plate）. Results All patients were followed up for 18.3 months averagely（range from 12 to 24 months）. Primary wound healing was achieved in all cases. The average time of fracture healing was 18.9 weeks（range from 12 to 28 weeks）. There was no nonunion or malunion. According to the Schatzker-Lambert＇s criteria for knee joint function evaluation, the results were excellent in 2 cases, good in 4 cases and fair in 1 case.Conclusion Double-plating fixation via a Y-shaped surgical approach can be an alternative choice of treating AO-type C3 distal femoral fractures, on account of its advantages of adequate exposure, anatomical reduction and rigid fixation.
