臂-踝脉搏波速度计算公式值得商榷

分析臂-踝脉搏波速度（ba PWV）计算公式。从主动脉弓分支处开始,流入锁骨下动脉与颈总动脉的血液方向与降主动脉流入下肢的血液方向是相反的。当脉搏波到达肱动脉时,在心-踝这一方向上的脉搏波传导距离远远要大于心-上臂的距离。用一个明显长于实际血管的距离去除以一个相对短的时间,必然会得出一个较快的PWV。统计分析表明,ba PWV不仅夸大了实际的脉搏波速度,而且还颠倒了真实的脉搏波速度。继续研究ba PWV只会给我们带来麻烦。因此,笔者建议,ha PWV应引起我们的重视。

Formula for brachial-ankle pulse wave velocity is worth discussing

Blood direction from the aortic arch branch into the subclavian artery and the common arotid arteries is opposite to the one from descending aorta into the lower limb. When pulse wave reaches the brachial artery, pulse wave transmission distance on the direction of heart-ankle is far greater than that on the heart-braehial direction. In the formula for brachial-ankle pulse wave velocity （baPWV）, the distance longer than the actual distance is divided by a relatively shorter duration, so the result PWV would be faster than the actual value. Statistical analysis shows that baPWV overstates not only actual pulse wave speed but also reverses the real pulse wave velocity. More studies on baPWV will only bring us trouble. Therefore, we suggest that heart-ankle pulse wave velocity should draw our attention.
