老年人胆总管结石超声引导下经内镜取石与外科手术治疗的比较

目的比较老年人胆总管结石在超声引导下经内镜取石与外科手术治疗的疗效。方法观察92例60岁以上胆总管结石的患者,其中43例患者为超声引导下经内镜取石治疗(内镜组),另外49例为外科手术治疗(手术组),比较两组治疗的效果。结果 43例超声引导下经内镜取石治疗治愈率为97.67%(42/43),49例经手术治疗组治愈率为93.87%(46/49)。超声引导下经内镜治疗残余结石发生率为0%(0/42),手术治疗残余结石发生率为6.12%(3/49)。黄疸消退时间内镜组为(5.05±0.12)d,手术组为(8.85±0.32)d,住院时间内镜组为(6.62±0.28)d,手术组为(25.67±3.47)d。结论超声引导下经内镜胆总管取石治疗创伤小、术后恢复快、并发症少,特别适合老年人胆总管结石治疗。

Comparison of choledocholith treatments effects between endoscope with ultrasound-guided and surgery in elder patients

Objective To compare the different effects between endoscope treatment by ultrasound and surgery in the old patients with the choledochus-stone. Methods 92 patients over 60 years old were observed,among which 43 in endoscope group and 49 in surgery group. Results The result showed that the cure rate was 97. 67% in endoscope treatment; 93. 87% in surgery,the rate of remaining stone was 0% and 6. 12% ; the time of jaundice disappear was 5. 05 ± 0. 12 days and 8. 85 ± 0. 32 days; the time of hospitalization is 6. 62 ± 0. 28 days and 25. 67 ± 3. 47 days. Conclusion The endoscope treatment guided by ultrasound is a safe and effective means to treat common bile duct stone,especially for the old patients.
