尿道损伤手术治疗及导尿管留置时间的临床观察

为探索骨盆骨折合并后尿道损伤的安全、有效治疗方法和留置支架导尿管的适当时间，我们对52例骨盆骨折伴后尿道损伤患采用简化的尿道会师牵引术及尿道造影术，对会师术后尿道愈合情况进行观察。结果表明10例(10％)在9周以内愈合，42例(81％)在10周以上愈合。愈合时间最短3周，最长16周。获得随访42例，随访1～9年，正常排尿31例(73．8％)。阳瘘1例。认为简化的尿道会师术，可作为首选方法治疗后尿道损伤，尿道造影可观察尿道愈合情况，确定支架导尿管拔除的最佳时间。

The clinical study of surgical treatment of urethral injuries and the optimal indwellins time of anurethral catheter

To investigate the treatment of pelvic tracture with urethral injuries and the proper indwelling time of an urethral catheter, pericatheter utethrography and simplified urethroplasty were performed repeatedly fur 52 cases of pelvic fractare with posteytor urethra injuries. The results show the healing up of the in-juried urethra needed 3 to 16 weeks, 19% of the patients requires less than 9 weeks, 81% of the patients requires more than 10 weeks 42 cases, had been followed up for 1 to 9 years and 31 or them couldnocmally urinate. It is suggested the simplified urethrorlasty can be used as prior treatments method for the urethral injuries and urethrography can be used as an indicator urethras healing and decision for the optimal time of urethral cathter extraction.
