急诊动脉栓塞对创伤性泌尿系统出血的疗效研究

目的全面分析和研究急诊脉栓塞对泌尿系出血的医治的效果和价值。方法收集2010年5月至2011年5月期间来我院治疗的泌尿系出血病患50例,在这50例泌尿系出血病患中有外伤性肾出血的病患27例、有经皮肾镜术后出血的病患13例、膀胱癌出血10例。结果 50例泌尿系出血病患中有非医源性肾损伤的病患27例,22例动脉栓塞之后第1d停止临床出血现象,血尿现象消失,3例术之后第4～6d血尿渐渐消失。13例经过皮肾镜术之后有出血临床现象的病患当中,8例病患的出血原因为穿刺误伤到小动脉出血,5例病患的出血原因为穿刺误伤到小动脉伴动静脉瘘所形成。10例膀胱肿瘤出血病患在动脉栓塞之后都在清除体内血块之后尿颜色呈现洗肉水样并渐渐转为清亮。结论泌尿系出血采用急诊动脉栓塞手术方法,是一种高效、主动、迅速和安全的止血方法,治疗效果明显,且可以最大限度地保留病患的肾脏功能。

Research on Curative Effect of Emergent Arterial Embolization on Traumatic Urinary Tract Hemorrhage

Objective To explore the clinical effect and value of emergent arterial embolization in the treatment of traumatic urinary tract hemorrhage. Methods 82 patients with traumatic urinary tract hemorrhage in our hospital i~om May 2010 to May 2011 were analyzed retrospectively. Results Among 50 patients, 27 cases were non-iatrogenic renal injury; bleeding in 22 cases were controlled one day after operation without hemaatria phenomenon; bleeding in 3 cases were controlled 4 to 6 day s after operation with hematuria gradual disappearance. Among 13 cases of hemorrhage after percutaneous nephroscopy, 8 cases were caused by arteriole injury, 5 cases were caused by arteriole injury and arteriovenous fistula. Conclusions Emergent arterial embolization in the treatment of traumatic urinary tract hemorrhage is an efficient, active, quick and safe haemostasis with obvious treatment effect, which can maximize the retention of the patients＇ kidney fimction.
