日间手术模式与加速康复外科在腹腔镜胆囊切除术患者中的联合应用

目的探讨日间手术模式与加速康复外科联合应用在腹腔镜胆囊切除术患者中的价值。方法回顾性分析2018年6月～2019年1月367例日间模式与加速康复联合应用的腹腔镜胆囊切除术(联合组)及425例常规模式的腹腔镜胆囊切除术(常规组),联合组使用本院制定的联合诊疗规范进行围手术期的管理。比较两组患者的一般资料、住院总费用、住院时间、并发症及患者满意度等并进行统计学分析。结果联合组延长住院24例(6.5%),中转开腹2例,手术均顺利完成,术后无严重并发症。联合组患者在住院时间、住院总费用、并发症发生率及患者满意度方面优于常规组,差异有统计学意义(P0.05)。结论腹腔镜胆囊切除术患者中日间手术模式与加速康复外科联合应用是安全和有效的,大大缩短了住院时间,加速患者康复,降低了手术风险及医疗费用,提升了医疗质量及就医感受,值得推广。

Combined application of day surgery mode and accelerated rehabilitation surgery in laparoscopic cholecystectomy

Objective To investigate the value of combined application of day surgery mode and accelerated rehabilitation surgery in patients undergoing laparoscopic cholecystectomy.Methods 367 patients with laparoscopic cholecystectomy combined with day surgery mode and accelerated rehabilitation surgery(combined group)and 425 patients with routine laparoscopic cholecystectomy with conventional mode(conventional group)from June 2018 to January 2019 were retrospectively analyzed.The combined group used the combined medical treatment guidelines developed by the hospital for perioperative management.The general data,total hospitalization costs,length of hospital stay,complications,and patient satisfaction were compared between the two groups and statistically analyzed.Results In the combined group,there were 24 cases(6.5%)of extended hospitalization and 2 cases of transferring to open surgery.The operation was successfully completed.There were no serious complications after operation.The hospitalization time,total hospitalization cost,complication rate and patient satisfaction in the combined group were better than those in the conventional group,and the difference was statistically significant(P<0.05).Conclusion The combination of day surgery mode and accelerated rehabilitation surgery in patients undergoing laparoscopic cholecystectomy is safe and effective,which greatly shortens the length of hospital stay,accelerates patient recovery,reduces the risk of surgery and medical expenses,and improves the quality of medical care and medical treatment.It is worth promoting.
