急性低频感音神经性听力损失和梅尼埃病眼源性前庭诱发肌源电位的差异性研究

目的分析急性低频感音神经性听力损失(ALHL)和梅尼埃病眼源性前庭诱发肌源性电位(oVEMP)的不同。方法选取我院耳鼻咽喉科2017年1月～2018年6月ALHL组45例,另选取梅尼埃病组36例,分别以500 Hz及1 000 Hz气导短纯音作为刺激声,对受试者进行oVEMP的检测。对两组患者的oVEMP的N1～P1波振幅及500 Hz/1 000 Hz频率振幅比进行差异性比较分析。结果 500 Hz oVEMP ALHL组异常率22.2%,梅尼埃组异常率25.0%,两组比较差异无统计学意义(P0.05)。1 000 Hz oVEMP ALHL组异常率26.7%,梅尼埃组异常率27.8%,两组比较差异无统计学意义(P0.05)。500 Hz和1000 Hz频率调谐性变化两组比较,ALHL组发生率6.7%,梅尼埃组发生率58.3%,两组比较差异有统计学意义(P0.05)。结论 ALHL与梅尼埃病两种疾病的oVEMP比较,由于迷路积水和椭圆囊均会受累。而oVEMP的频率调谐性变化对梅尼埃病有诊断意义。

Study on the differences between acute low-frequency sensorineural hearing loss and ocular vestibular evoked myogenic potential in Meniere's disease

Objective To analyze the differences between acute low-frequency sensorineural hearing loss(ALHL)and ocular vestibular evoked myogenic potential(oVEMP)in Meniere''s disease.Methods From January 2017 to June 2018,45 patients in the ALHL group and 36 patients in the Meniere''s disease group in the E.N.T.Department in our hospital were collected.The short tone burst of 500 Hz and 1000 Hz was used as the stimuli,and the subjects were given the test of oVEMP.The differences in N1-P1 wave amplitude of the oVEMP and the frequency amplitude ratio of 500 Hz to 1 000 Hz were compared between the groups and analyzed.Results For 500 Hz oVEMP,the abnormal rate in the ALHL group was 22.2%,and the abnormal rate in the Meniere''s disease group was 25.0%.There was no statistically significant difference between the two groups(P>0.05).For 1000 Hz oVEMP,the abnormal rate in the ALHL group was 26.7%,and the abnormal rate in the Meniere''s disease group was 27.8%.There was no statistically significant difference between the two groups(P<0.05).The frequency tunability changes of 500 Hz and 1000 Hz were compared between the two groups,and the incidence rate was 6.7%in the ALHL group and 58.3%in the Meniere''s disease group.The differ ences between the two groups were statistically significant(P<0.05).Conclusion The oVEMP is compared between the two diseases of ALHL and Meniere''s disease,and the alveus communis is affected due to hydrolabyrinth.The frequency tunability changes of oVEMP have diagnostic significance on Meniere''s disease.
