老年脑梗死患者下肢动脉病变的彩色多普勒超声观察

目的应用彩色多普勒超声对探讨老年脑梗死与下肢动脉硬化的关系。方法对57例老年脑梗死患者与33例无脑梗死患者进行下肢动脉彩色多普勒超声检查，观察动脉内膜回声及有无粥样硬化斑块，测量中-内膜厚度（IMT）、斑块的大小、管腔是否狭窄及狭窄程度。结果脑梗死组较对照组膝以上的动脉（股总动脉、股浅动脉）粥样硬化斑块发生率增高（78．9％vs47．0％，65．8％vs45．0％，均P〈0．01）；股总动脉、股浅动脉Ⅰ级狭窄发生率增高（60．5％vs33．3％，P〈0．01；47．4％vs30．3％，P〈0．05）；股总动脉、股浅动脉IMT增厚（1I51±0．53）mm vs（1.27±0．53）mm，P〈0．01；（1．37±0．50）mmvs（1．20±0．45）mm，P〈0．05，粥样硬化情况更为严重。结论老年无糖尿病脑梗死患者膝以上动脉粥样硬化病变发生率增加，脑梗死与膝以上动脉粥样硬化有关，下肢动脉超声检查可间接反映脑动脉硬化的情况，预测脑梗死发生的风险。
