2009—2011年恩施州HIV初筛阳性标本确证实验结果及带型分析

目的对2009—2011年恩施州186份人类免疫缺陷病毒(HIV)确证阳性样本进行免疫印迹(WB)条带分析,增强对WB试验结果的分析判断能力。方法采用酶联免疫吸附试验(ELISA)对2009—2011年恩施州辖区HIV抗体初筛阳性标本进行复核检测和使用WB试验进行确证检测。结果 269例初筛阳性者中ELISA复核HIV抗体阳性210份,阳性率为78.07%;复核HIV抗体阴性59份,阴性率为21.93%。WB试验阳性186份,占88.57%;15份为不确定,占7.14%;9份为阴性,占4.29%。WB带型分布:其中7条带以上共计181例,占97.31%;6条带的3例,占1.61%;5条带的1例;占0.54%。性别、不同年龄组间与各WB带型分布间的差异无统计学意义。结论在艾滋病HIV抗体初筛与确证实验中,复检试剂种类的选择对确证阳性率影响较大,WB带型的阳性判定应严格按照《全国艾滋病检测技术规范》,并结合试剂说明书,参考实际情况,谨慎判定。

Confirmatory experiment results and banding pattern analysis of HIV screening positive samples in Enshi Autonomous Prefecture from 2009-2011

Objective] To perform WB banding pattern analysis on HIV 186 confirmatory positive samples in Enshi Autonomous Prefecture from 2009-2011,and enhance the ability of analyzing and judging the WB experimental results.Methods]The method of enzyme-linked immune response（ELISA） was used to review and test the HIV screening antibody positive samples from medical units,and then immunoblotting test（WB） was adopted to perform confirmatory test on positive ones in Enshi from 2009-2011.Results] There were 210 ELISA reviewed positive cases of 269 screening positive cases,the positive rate was 78.07% and 59 cases were negative with negative rate of 21.93%.And from the results of WB test,there were 186 WB confirmed positive cases of 210 reviewed positive cases,accounting for 88.57%,and 15 cases were uncertain,accounting for 7.14%,9 cases were negative,accounting for 4.29%.From the distribution of WB banding pattern,there were 181 cases with 7 strips and above,accounting for 97.31%,and 3 cases with 6 strips,accounting for 1.61%,1 case with 5 strips,accounting for 0.54%.There was no statistical difference of the WB type distribution between genders and different age groups.Conclusion]In the HIV antibody screening and confirmatory experiment,the selection of the type of recheck reagent has great Influence on confirmed positive ratio.Positive judgment of WB banding pattern should be strictly in accordance with the＂ National HIV Testing Technical Specifications＂ and judgment should be made carefully by binding the reagent specification and referencing the actual situation.
