全反式维甲酸联合三氧化二砷治疗急性早幼粒细胞白血病临床观察

目的探讨全反式维甲酸（ATRA）联合三氧化二砷（As2O3）治疗初治急性早幼粒细胞白血病（APL）的疗效和不良反应。方法初治APLl4例均接受ATRA联合As2O3方案治疗，ATRA25mg／m^2／d，分2—3次口服；As2O3 10mg／m^2／d加入5％葡萄糖500ml静脉滴注持续3—4h，1次／d，观察CR率，CR时间以及不良反应。结果14例患者完全缓解（CR）13例，死亡1例，CR率92．9％，达CR时26-45d，平均30．1d。结论ATRA联合As2O3治疗初治APL疗效好，不良反应少，能缩短达CR的时间。

Clinical observation of the combination of all-trans retinoic acid and arsenic trioxide for the treatment of acute promyelicy leukemia

Objective To observe the efficacy and side effect of the all-trans retinoic acid （ATRA）and arsenic trioxide （ As2O3 ） combination in acute promyelicy leukemia. Methods Fourteen APL patients were treated with ATRA combined with As2O3. The treatment protocol was as following ：10 mg/m2 As2O3 in 500 ml glucose solution forintravenous drip over 3 to 4 hours once a day. ATRA was given 25 mg/m^2 every day （ in two or three separate doses）until complete remission（ CR）, then observed the efficacy, adverse effects, the CR rate and the duration for achieving CR. Results 13 patients reached CR, The CR rate was 92. 9 %. The time of achieving complete remission by ATRA combined with As2O3 was 26-45 days and median time to achieve CR was on 30. 1 days. Conclusion The combination of ATRA and As2O3 in the treatment of APL can obtain a higher CR rate without intolerable side effects and a shorter duration for achieving CR.
